Job summary

The post holder will required to have a high level of relevant knowledge and skills, which you will use to manage, develop and care for pregnant women with complex vulnerabilities in partnership with the wider multidisciplinary team.

The post holder will work flexibly within the Specialist Midwifery Team to provide caseholding care for the most vulnerable and complex clients .

To contribute to the development and delivery of high quality midwifery services in line with professional and government policy in order to tackle the negative impact on pregnancy associated with Vulnerable Migrant Status. You will provide a seamless service that is responsive to client need. This will involve delivering care from both acute sites and the community.You will ensure that relevant pathways, policies and processes for women with these additional vulnerabilities are followed whilst keeping up-to-date with local and national guidelines related to this specialist role.

Main duties of the job

  • You will be responsible for a caseload of vulnerable women with complex social needs, specifically vulnerable migrant women, Your role will include the planning and implementation of individualised midwifery led care offered in partnership with the woman and the multidisciplinary team.

  • Maintain a safe working environment for own professional practice as well as the wider team.
  • Fulfil Trust health and safety policies and procedures when performing risk associated procedures including dealing with hazardous substances
  • Adhere to and encourage others to utilise appropriate NMC and local standards for record keeping and all written documentation, ensuring client and colleague confidentiality at all times.

Job responsibilities

Clinical

  • You will be responsible for a caseload of vulnerable women with complex social needs, specifically vulnerable migrant women, Your role will include the planning and implementation of individualised midwifery led care offered in partnership with the woman and the multidisciplinary team. Using continual assessment and analysis you will provide continuity of care throughout the womans pregnancy, birth(where appropriate) and the postnatal period to ensure her medical, psychological and social needs are assessed and holistically cared for.

  • You will be working alongside multi professional teams providing the most appropriate care for women whilst breaking down barriers that can inhibit women with socially complex lives accessing high quality maternity care. Working in a variety of settings including community, hospital and womens homes including confidential addresses.

  • Have an in depth understanding of barriers which may impact or inhibit women from accessing timely and regular antenatal care.

  • Have increased knowledge of and interest in, the impact that immigration status and ethnicity has on pregnancy outcomes. Have awareness and knowledge around other complexities including domestic abuse, homelessness and poor mental health. Work in partnership with other members of the Specialist team to ensure a joint plan of care and management of risks

  • Have sound knowledge of safeguarding procedures and access regular safeguarding supervision.

  • Promote both short-term and long-term health through evidence-based education. Support women and their families to make sustainable changes to improve health outcomes and decrease the generational impact.

  • Provide and perform accurate assessments and procedures during the antenatal period, labour care and in the postnatal period.

  • Support womens choices regarding feeding their infant, whilst encouraging and promoting UNICEF Baby Friendly Initiatives.

  • Administer medications under the scope of Midwifery Exemptions/Patient Group Directives when required or appropriate.

  • Participate in and support the development of evidence based clinical practice, ensuring agreed policies, guidelines and standards are implemented and monitored.

  • Develop individualised care pathways for vulnerable women and families affected by complex needs, supported by the specialist team.

  • Have up to date knowledge of local services available to pregnant women in relation to their mental, physical and social needs.

  • Provide a visible and accessible presence, acting as a professional role model working within multidisciplinary and midwifery teams.

  • Link and liaise with the Safeguarding Team and Named Midwife for Safeguarding on safeguarding issues to recognise and respond appropriately and effectively to concerns relating to both children and the unborn baby.

  • Contribute to audit processes that are in place within The Specialist Midwifery team.

  • Contribute to policies and guidelines relating to women with complex needs and support colleagues to access and adhere to the policies.

  • Contribute and participate in providing evidence based training for staff in line with national and local policies.

  • Participate in the induction of new midwifery staff, sharing your knowledge and upskilling them.

Provide evidence-based advice, support and information with regards to the appropriate management and referral of women with complex needs and their families to appropriate services

  • Seek advice and support from the Line Manager and the Band 7 midwives in The Specialist Midwifery Team whenever necessary. Working closely and in conjunction with Band 7 Midwife for Vulnerable pregnant migrants.

  • Maintain professional conduct including appearance at all times.

  • Act as a resource for all staff involved in maternity care (including obstetricians, midwives, student midwives and maternity support workers). Provide evidence-based advice, support and information with regards to the appropriate management and referral of pregnant women with complex needs.

Work with colleagues to ensure that there is awareness and knowledge about pregnant women with social complexities and their needs are identified and risks mitigated using appropriate support and referral pathways

Support and assure the implementation of guidelines by statutory and professional bodies when delivering midwifery care to women with complex needs.

Use research to support evidence-based practice

Contribute to the wider public health agenda by delivering a flexible high quality midwifery service and contribute to service evaluation and implementation of quality improvement initiatives.

To improve the experience of childbirth for Nottingham women offering choice of place of birth and involving women in decisions regarding their care

Knowledge & Communication

  • Ensure processes are in place to facilitate effective communication with all disciplines, patients and relatives that meets individual needs.
