You will work alongside an established team of Clinical Nurse Specialists to deliver high quality patient care and support for health care professionals
Main duties of the job
- Management of anticoagulation for patients on vitamin K antagonists
- Running a morning clinic Monday - Friday at QMC campus
- Providing a ward discharge service for patients on vitamin K antagonists across both City and QMC campus.
- Arranging appropriate Anticoagulation for patients referred post USS scan with Deep vein thrombosis
- Providing support and telephone counselling to patients who are discharged following a Pulmonary Embolism
- Offering a patient self-testing training service using near patient INR testing

The Anticoagulation team is a small established team of approx 16 staff including clinical nurse specialist , pharmacist and clerical staff
The team are hardworking, enthusiastic, dynamic and committed to providing outstanding patient care. Service development is ongoing within anticoagulation and we embrace change to improve the care we provide. we take part in regular audits and we are currently taking part in a national research study. we have recently had some of our work published nationally and also by NICE
