A new exciting opportunity has arisen for an enthusiastic, motivated and dynamic Clinical Pharmacist to support in delivering enhanced Clinical Pharmacy services to the highly specialist areas: Clinical Immunology, Rheumatology and Dermatology. The role also involves providing high quality pharmaceutical care to the Trust's Transfer of Care Unit"(TOCU).
This unique role provides the opportunity to work alongside highly specialist pharmacists to develop your clinical knowledge and expertise and to participate in specialist clinics; i.e. Drug Allergy clinics.
You will develop competence in, and support with the management of patients with a wide range of immunological conditions, rheumatic diseases, allergic conditions and dermatological conditions. You will also support with the screening of Homecare prescriptions across the specialties.
Additionally you will work collaboratively with the "Transfer of Care" MDT to improve timely provision of discharge medications.The QMC TOCU accepts patients from a range of specialities and you will play a vital role in improving the quality of discharge communication and facilitating the smooth transfer of medication related issues across the interface. Ensuring patients are discharged from hospital with the right medication information and receive the right onward support by ensuring appropriate referrals for medication reviews for high -risk and complex patients.
Main duties of the job
The post will be based predominantly at the Queens Medical Centre Campus, but there may be the expectation to travel to the City campus on occasions.

To provide a clinical pharmacy service to the new "Transfer of Care Unit", offering highly specialist pharmaceutical advice to medical staff, nurses and patients on all aspects of safe and effective use of medicines.
To support with the clinical screening of cDMARDs, biologics and specialist Rheumatology infusions .
To assist the Highly Specialist Pharmacist in delivering Pharmacist -led Drug Allergy Testing clinics.
To work alongside the multidisciplinary team delivering excellent patient care through medicines optimisation. When qualified the post holder will also utilise their prescribing skills to do this.
To facilitate with timely discharges by clinically screening discharge prescriptions/TTOs. To be the lead pharmacist for the "Transfer of Care Unit".
To improve the quality of discharge communications between secondary and primary care including care homes.
Liaise closely with Community Pharmacy Services, PCN Pharmacists and G.P. Practice Pharmacists to ensure appropriate referrals of Medication Reviews for high -risk patients with complex medication related needs.
To support, mentor & line manage junior pharmacists and medicines management technicians.
