Job summary

This is an exciting and challenging opportunity for an individual who is passionate about making a difference to patients, supporting a busy team and able to support and implement change. We welcome applicants who are professionally registered from either Nursing or AHP backgrounds.

NUH and the Integrated Discharge Team are looking for a dynamic team manager who will need to engage with ward and therapy teams, community providers and social care. You will have strong links with the Transfer of Care Hub and provide an effective interface between the wards and the hub to support safe quality discharges for our patients.

Main duties of the job

As Team Manager you will provide daily leadership and support to the team of clinicians, support staff and administrative staff. You will manage staff sickness, recruitment, performance, appraisals and training using the e-roster system to plan and manage service delivery.

You will provide expert clinical support to the team and also to wards around complex discharge, having a broadknowledge of the Discharge to Assess model and be able to communicate effectively to colleagues, patients and families the processes and plans for discharge.

You will need excellent communication skills, the ability to understand complex reports and data, be able to use various ICT systems including patient information systems, and be able to process information quickly to make decisions.
