Job summary
The National Paediatric Ataxia Telangiectasia (A-T) clinic is based at Nottingham and is responsible for co-ordinating the long-term multi-disciplinary care of over eighty patients with A-T, an autosomal recessive, neurodegenerative, life-limiting genetic condition. The clinic is held several times a year at Nottingham, with families attending for full-day clinics from across the UK and Southern Ireland.Families see a range of professionals including Neurology, Respiratory, Speech and Language, Dietetics, Genetics, Immunology, Occupational Therapy, Physiotherapy and Psychology.The Lead Nurse is responsible for the overall planning running of the multidisciplinary clinics, including information gathering beforehand; liaison with the A-T Charity and ensuring that recommendations are subsequently followed up at a local level. Some aspects of care benefit from further follow up and continued contact with families happens throughout the year. The Lead Nurse will work closely with the Clinical Lead to manage patient pathways, implement new ways of working and to support in the training and education in new and other clinic staff.The post holder is required to be registered with the Nursing and Midwifery Council.
Main duties of the job
Liaise with the Band 4 A-T PA to plan clinic attendance; manage timetabling to ensure patients see all specialities with best use of resources.Liaison with the A-T charity to support patient queries and to ensure accommodation is in place for patients attending clinicLiaison with the A-T adult team at Papworth to ensure effective transitioning of care from paediatric to adult servicesEnsure that MDT meetings and National A-T audit meetings are scheduledSupport A-T families who contact the service to receive support and advice and ensure relevant follow-up takes placeMaintain accurate and up to date confidential patient hand-held and electronic recordsWork with Band 4 A-T PA to ensure that multi-author clinic letters are distributed following clinic visitsTo work with colleagues in the Division to provide seamless and effective care management
Support GP's and other HPs to manage patients' care plans locally
In partnership with other colleagues, participate in the development of care pathways withinarea of responsibilityContribute to the implementation of the Division clinical governancewith respect to the A-T clinic
