Job summary

We have created an exciting opportunity for a Clinical Specialist Speech and Language Therapist in Frailty. This 0.6wte band 8a post will be a new role working within an innovative and research lead Health Care of the Older Person service and Speech and Language Therapy Service. The post holder will work with the wider multi-professional team focusing on the frailty pathway. As a new role the post holder will be able to influence it's scope and development. They will be well supported by an excellent team with leadership and management support. This is a very exciting opportunity!

The Speech and Language Therapy services at NUH have recently undergone a significant transition. The Speech and Language Therapy service is now embedded with the other AHPs in the Therapies & Rehabilitation directorate. This has created increased levels of engagement and has provided exciting opportunities around how the adult acute service is delivered. The service is operationally managed by Speech & Language Therapists and benefits from the support available in Therapies around workforce development, training, and developing clinical roles and advanced practice.

Main duties of the job

The post holder will work alongside the senior SLT leadership team to influence the strategy of how we deliver our services. The post holder will work within the Specialist Medicine & Dementia team. They will lead our work on the Health Care of the Older Persons admissions ward, building relationships and supporting decisions around patient admissions and discharges. They will work along the frailty pathway, working to prevent decompensation and to support those at the end of their lives. They will also have the opportunity to use and further develop their skills in the use of instrumental assessments for this client group.

Applicants should have excellent organisation skills and be able to manage a caseload and set priorities. They will provide supervision and second opinions to less experienced members of staff. They will work closely with the multi-professional teams at NUH. They should have advanced skills working with swallowing and communication difficulties.

They will need excellent communication and leadership skills, be adaptable and confident to work autonomously and with other members of staff within a supportive team.

We are looking for an SLT who is enthusiastic, motivated, able to evidence continuing professional development and keen to develop themselves and others.

NUH is a large teaching hospital, across 2 sites: Queens Medical Centre and City Hospital. The Speech & Language Therapy service works across both sites, working alongside a separate ENT service. We work within a very broad range of acute specialties including Healthcare of the older person, Respiratory, and Critical Care. NUH host a number of regional services including Neurosciences unit, Major Trauma Centre, a comprehensive Stroke unit, Trent Cardiac centre, Burns Unit and Upper GI services. We also have SLTs within the Neuro-rehabilitation unit, the MDT neuro-rehabilitation outpatient services, and an SLT service for Neurology outpatients.

To provide clinical leadership in the management of patients under the care of NUH within a specialist area. vice

To develop and extend the scope of practice through undertaking research projects and implementing evidence-based practice.

KEY JOB RESPONSIBILITIES

To provide clinical leadership within the specialist service, through consultation and joint working with all bands of staff, in the management of complex patients, this will involve review of their clinical reasoning, therapeutic handling and treatment skills.

To maintain and develop high standards of patients care and facilitate and implement evidence based practice by ensuring implementation of standards set in relevant national guidance, specialist clinical group guidance and specialist relevant areas. Also by ensuring staff are informed of recent developments through attendance at research based conferences, relevant network meetings and postgraduate education and training courses.

To develop local pathways of care for specialist area patients

To evaluate the effectiveness of the service through service evaluation & clinical audit in liaison with the Therapy Services Manager and relevant stakeholders.

Where appropriate, offer extended scopes of practice so as to facilitate the optimal management of patients within a clinic setting (e.g. X ray ordering, prescribing)

To ensure waiting list targets are maintained by initiating and developing new ways of working (e.g. telephone consultation, opt-in appointments, SOS appointments).

To ensure high standards of clinical care within the section on issues pertaining to specialist area to own team and a wide range of disciplines (e.g. medical staff and the MDT) in line with the education and training strategy by:

  • In-service training and development programme
  • One to one coaching / teaching
