Job responsibilities
To support clinical pharmacists by participating in the provision of a high quality, professional and effective medicine management service across Critical Care and Theatres at QMC:
Leading the technical clinical pharmacy service to critical care, theatres and enhanced post-operative care units.
Providing specialist level medicines management support to the service.
Leading on the training of the team of MMTs and Medicines Management Assistants working in the specialty.
Developing and implementing Standard Operating Procedures relating to pharmacy processes within critical care, theatres and the enhanced post-operative care unit to ensure a high quality service is provided.
Leading on the management of the Omnicell medicines storage solutions, including assurances for effective stock control, storage and security.
Taking drug histories from patients on admission to hospital:
by checking patients own medication, their medical notes, Medway, SystmOne, GP surgeries, recent discharges and any other appropriate sources of information.
ensuring that the medication history obtained is documented clearly & correctly as per pharmacy procedures.
accurately communicating issues back to the relevant pharmacist.
Communicating effectively, clearly and concisely complex or sensitive information to a wide range of staff, healthcare professionals and patients where there may be barriers to understanding.
Checking patients own medicines for suitability for use on admission and at discharge.
Checking the transcription of medication for discharge on the eTTO before the Pharmacist has seen the discharge prescription.
Carrying out final accuracy checks of discharge prescriptions against contents of patients own drugs locker.
Maintaining up to date clinical handovers on NerveCentre.
Being responsible for the supply of items for one-stop dispensing (and self- administration where appropriate) checking the patient locker to see what supplies are still available on the ward.
Highlighting the need for any additional compliance aids and supply if necessary to assist with medication compliance.
Liaising with GPs, community pharmacies and other primary care professionals to ensure safe use of medicines and continuity of care is maintained.
Participating in the full programme of validated checking and training schemes in accordance with agreed protocols.
Liaising with ward and pharmacy staff to resolve any technical queries or problems with the patients, refers clinical queries to the pharmacist for resolution.
Assisting the Medicines Information department in the co-ordination of the Drug Hazard/Recall procedure in the event of a drug recall, where appropriate.
Representing the Pharmacy Department when working in ward, clinic or other departmental environments and upholding the standards expected.
Participating in ward based audit schemes or other forms of research and development.
Assisting the Critical Care and Theatres Pharmacy Team in the management and monitoring of controlled drugs. Participating in the controlled drug retrieval of patients own medication from wards, and expired or unused ward stock. Checking CD register balances and balance transfers to new CD registers. Helping support the regular ward based CD audits and the Trust wide routine CD audits.
Being involved in the training of new clinical pharmacy ward staff and the revalidation process of existing MMTs.
Participating in the NVQ Level 3 Clinical training and assessment process of Student Technicians at ward level.
To be responsible for the local implementation of standards relating to medicines storage and security including:
Ambient and fridge temperature monitoring.
Security of medicines storage in treatment rooms, drug trolleys, fridges, emergency trolleys and boxes, controlled drugs cupboards and Omnicells .
To participate and support in project works under the direction of the Divisional Lead Pharmacist.
Other Duties
Taking line management responsibility for designated staff, this is to include:
Personal development reviews.
Conduct, Performance and sickness management.
Recruitment and selection.
Being responsible for ensuring appropriate levels and skill mix of technical and support staff are available to maintain agreed standards of service delivery for a defined range of service users through the management of annual leave, TOIL, study time etc.
Taking a lead role in the organisation, delivery and development of the ward technician training with the critical care and theatres team, including ensuring adequate training packages are in place.
Adhering to relevant legislation, such as the Medicines Act, GMP, H&S, COSSH, etc. and to Trust policies.
Practicing at all times, in accordance with the Code of Ethics of the General Pharmaceutical Council.
Taking responsibility for your actions as a registered pharmacy technician.
Ensuring safe working systems are adopted and adhered to.
Maintaining confidentiality in all work areas.

Demonstrating commitment to continuing professional development and maintaining a portfolio of practice.
Undertaking annual mandatory training e.g. manual handling, fire, security, basic life support etc.
Undertaking any other duties relevant to the grade as specified by the Chief of Pharmacy and Medicine Management.
General Pharmacy Department
To contribute to departmental procedures at the request of the Policies and Procedures committee.
To undertake project/audit work as required.
To participate in rostered flexible shift working including evening, weekend and bank holiday arrangements in line with service commitments. The trust is working towards a 7 day 24 hour service and at present the hours can be between 8am and 8pm.
To undertake any other duties that may reasonably be required.
