This is a substantive post to support cardiac implantable device procedures within NUH and growth of our heart failure service.The post will also contribute to the acute cardiology service providing additional support for the increased number of patients admitted with acute general cardiac conditions and deliver cardiology (general and specialist) outpatient clinics. The successful applicant will have broad experience in all aspects of clinical cardiology, with particular expertise in complex device therapy. The appointee will participate in the cardiology on-call rota, teaching programmes and will be eligible for Clinical Teacher status in the University of Nottingham.Most activity will be delivered within the tertiary centre at NUH. The appointee will participate in the Consultant of the week rota rotating through the Acute Cardiac Unit and in-patient wards currently on a 3:20 rotation. They will also participate in the QMC rota. Weekend on-call is 48 hours from Saturday 9am until Monday 9am also on a 1:10. On-call frequency may change with future appointments and service improvements. Time off in-lieu is given for weekend working.

Main duties of the job

Applications are invited for the post of Consultant Cardiologist (devices/heart failure) available from October 2022. This is a permanent post.

The successful applicant will join a team of 20 full time consultant cardiologists. They will contribute to a comprehensive consultant-led secondary and tertiary cardiology service primarily based in the Trent Cardiac Centre, Nottingham University Hospitals NHS Trust. They will undertake outpatient clinics and ward rounds to supervise care of inpatients admitted under the cardiology service and provide a specialist consult service to other inpatients when on call. They will undertake device implantation and supervise review of devices. There is also a requirement to contribute to the acute medicine service with an evening review shift 10 times per year. They will contribute to the non-interventional cardiology on-call rota which is currently 1:10 but may be subject to change.

The consultant cardiologists at Nottingham University Hospitals provide secondary and tertiary cardiology services. The tertiary catchment covers Nottingham and surrounding districts, North Nottinghamshire, and parts of Lincolnshire and Derbyshire (population approximately 1.3 million). Secondary cardiology services in Nottingham are provided over both campuses with the majority of acute cardiac admissions through the Emergency Department based on the QMC campus.

The Trent Cardiac Centre (TCC) is a purpose built three storey build that opened in 2005 providing two new cardiac catheter laboratories with a day case unit and facilities for day case echocardiography. The building also includes two cardiac surgical theatres, dedicated intensive care and high dependency units with combined cardiology and cardiac surgical ward accommodation. An additional two cardiac catheter laboratories used for diagnostic angiography, cardiac pacing and cardiac electrophysiology are based in the radiology department.

The successful applicant will join a team of 18 full time consultant cardiologists and will contribute to a comprehensive consultant-led secondary and tertiary cardiology service. Consultant cover for QMC campus is currently round the clock.The in-patient cardiology service is wholly based on the Nottingham City campus and consists of a 20-bed Level-2 Acute Cardiac Unit based on Morris Ward in the Trent Cardiac Centre (TCC). There is a daily Consultant ward round supported by a Specialist Registrar and more junior ward Doctors (CT, F2 and F1) in Cardiology across all our in-patient ward areas. Cardiology Consultant and Registrar level cover is provided every day around the clock. The remainder of Morris ward contains cardiac surgical patients. A further 32 Level-1 cardiology beds are located on Papplewick Ward and 22 low dependency, non-monitored beds on Patience 1 Ward which also houses our newly opened 4 bay ambulatory heart failure unit. Patients undergoing procedures are admitted to the 8 bedded short stay unit based in the Trent Cardiac Centre.The Trent Cardiac Centre (TCC) is a purpose built three storey build that opened in 2005 providing two new cardiac catheter laboratories with a day case unit and facilities for day case echocardiography. In 2014 we started sub-cutaneous ICD implants which are now mostly performed with serratus anterior plane block and conscious sedation; and in 2015 leadless pacemaker implants and His bundle pacing. There were 114 ILR implants and as of 2018 this is an Allied Professional led activity. We have also developed an Emergency Pacing Pathway, whereby patients with high grade AV block and asystole are admitted directly by ambulance to the Trent Cardiac Centre and can potentially undergo emergency permanent pacing if appropriate.Cardiac electrophysiology with ablation is performed on the NCH campus. The NUH atrialfibrillation program began in 2009 and we have an actively growing VT ablation programme.Cardiac surgery was established at Nottingham City Hospital in late 1995 and now provides a comprehensive adult cardiac surgical service to mid-Trent. There are 4 cardiac surgeons and 5 cardiac anaesthetists. In 2019, 570 cardiac surgical procedures were undertaken in NUH. The cardiac surgical department holds monthly mortality and morbidity meetings and publishes an annual report, which is available on the hospital website.ClinicalThe post holder will participate fully in the departments activities, including inpatient and outpatient work. A full range of cardiology emergency and elective patients are admitted to Nottingham University Hospitals NHS Trust and the appointee will be responsible for the management of those patients admitted under his/her care irrespective of sub speciality.Work with consultant colleagues to provide a comprehensive elective and emergency Cardiology service including reciprocal cover for periods of leave.
