CONSULTANT GASTROENTEROLOGIST

Nottingham University Hospitals is one of the largest tertiary NHS Trusts in the country and is a referral centre for the Mid Trent Cancer Network, which covers a population of over 2 million. An exciting opportunity has arisen to join our cohesive and progressive gastroenterology and hepatology department which has a strong clinical and academic national profile (nddcbru.org.uk).

The appointee will be fully trained in the management of patients with acute and chronic gastroenterological conditions and in managing acute GI emergencies, including GI bleeding, and able to contribute to the acute GI on-call service at the Queens Medical Centre (QMC) and Nottingham City Hospital (NCH) campus, currently as part of a 1:14 consultant-led, specialist nurse and registrar supported, 24 hour rota. The full-time post is based on a 10 Programmed Activity (PA) job plan which does not include general medicine.

Main duties of the job

Provision of emergency gastroenterology service: NUH Acute Gastroenterology Service wasreconfigured to receive all acute gastroenterology patients at QMC campus from 2006. The appointee will contribute to this service, leading the team including specialty trainee registrars and nurses; providing assessment and planning management for acute admissions with GI and liver problems.

The team will also liaise with endoscopy staff to organise and perform urgent inpatient endoscopy procedures either in endoscopy or theatres with full anaesthetic support. The acute gastroenterology service at QMC is provided by gastroenterology consultants (1 in 13, including this new post) who provide 24 hours cover for this service, with the week split 4:3 (Monday-Thursday and FridaySunday). The consultant on-call, specialist nurse and StR will be free from other clinical commitments for that period.

About us

The Gastroenterology, Nutrition and Hepatology inpatient beds are located at the QMC campus. Emergency inpatient endoscopies, ERCP, EUS, EMR & RFA are provided from the 2 room endoscopy suite at QMC (currently relocated to theatres). Out-patient clinics and elective endoscopy (4 rooms) are delivered from the Nottingham Treatment Centre on the QMC site.

Although we do not have any in-patient Gastroenterology/Hepatology facilities on the City Hospital (NCH) Campus, we provide senior GI, Nutrition & Hepatology consultations for other specialty inpatients, and out-patient Gastroenterology, Hepatology, Endoscopy and Nutrition services. The 4 room Endoscopy unit at NCH provides elective and emergency diagnostic and therapeutic endoscopy facilities including complex polypectomy and transnasal endoscopy. It is a designated bowel cancer screening centre and recently introduced Bowelscope. Theatres are currently being used whilst Covidrelated ventilation work is underway.

There is an exciting opportunity to appoint a replacement full time consultant Gastroenterologist to join our dynamic team of Gastroenterologists.

The post holder is expected to have broad experience of gastroenterology and endoscopy,contributing to the core Gastroenterology elective and emergency service. Due to the size of the Gastroenterology service at NUH we are able to offer a bespoke opportunity to develop the job plan to match a particular area of expertise and enthusiasm in the successful applicant.

The post holder will have an annualised job plan that will include their own dedicated weekly sessions but allow them to work flexibly with other luminal gastroenterologists and the emergency gastroenterology service, avoiding the need for prospective cover. The post holder will have designated administrative support and work across both Queens Medical Centre and Nottingham City Hospital campuses. In-patient duties, based at the QMC, are shared between the consultants in 1 month blocks and supported by our 8 specialty registrars.

Out-patient clinics take place at both the City and QMC Campus. Booking rules for out-patient clinics are set by the individual consultants based on a point system (16-18 points per 4-hour session, 2 points per new & 1 point per follow up patient). We expect active discharge of patients to their primary care physician wherever possible; however, we acknowledge that as a tertiary centre of expertise some complex patients will need life-long specialist follow up and monitoring of new: follow up ratios is not always appropriate. Transformation work is underway to facilitate patient activated follow up of chronic conditions which will be reflected in out-patient case-loads.
