An exciting opportunity has arisen to develop and deliver prehabilitation and rehabilitation pathways across Barnsley and help thousands of patients across South Yorkshire.

Active Together is a prehabilitation and rehabilitation service supporting people following a cancer diagnosis with physical activity, nutrition, and wellbeing guidance. The service has been designed by Sheffield Hallam University who work in close collaboration with NHS teams to support the delivery and evaluation of the service.

The Active Together service launched in Sheffield in 2022 and is currently expanding to new sites across South Yorkshire in Barnsley, Rotherham and Doncaster. Active Together South Yorkshire will provide equitable, multi-modal and personalised prehabilitation and rehabilitation support that meets the needs of patients across the region and generates evidence for a commissioned service.

Barnsley Hospital Foundation Trust and Barnsley Premier Leisure are working in partnership to deliver this programme to the population of Barnsley.

Active Together is funded by Yorkshire Cancer Research, Yorkshires independent cancer charity, working so more people can live longer, healthier lives, free from cancer.

Main duties of the job

To provide therapy interventions and implement programmes of care devised predominantly by the specialist Physiotherapists and Dietitian. Within your scope of practise to be able to modify care plans. With the availability of registered staff as needed.

To promote and understand the impact of effective health promotion, empowering healthy lifestyles including nutrition and fluid balance.

The post-holder will manage a caseload which may span across the two professions and include the delivery of exercise and basic education sessions in relation to health promotion.

The post holder will have a passion for exercise and exercise benefits, they will motivate others to adopt healthy behaviour changes.

To work across the cancer pathway to provide pre-habilitation therapy focused support for patients about to undergo cancer treatment and post cancer treatment.

To support the dietitians and physiotherapists to deliver high standards of coordinated, personalised care whilst working as an integral member of the Active Together Team.

To work within the Active Together Cancer MDT including Clinical Nurse Specialists, support workers, surgeons, oncologists, Physiotherapists and Dietitians promoting effective inter-professional and multi-disciplinary team working within the scope of the role.

To provide therapy support to people at every stage of their cancer journey, this may include signposting them to other services with support from registered staff.

  • Current full driving license

  • Completed recognised IT course e.g. ECDL/Elite/MOST
  • Level 4 Exercise in Cancer qualification.
  • Level 4 Cardiac Rehabilitation qualification.
