3576 - Discharge Coordinator

We are looking to recruit an enthusiastic, well motivated team member to work within the discharge and patient flow team to support ward staff to facilitate safe and effective discharge from a ward area. Must have excellent communication skills, willing to learn new skills, have some understanding of safe discharge, able to liaise with all multi disciplinary teams.

Main duties of the job

The role of the discharge coordinator is to support ward staff to coordinate and facilitate safe and effective discharge, working alongside ward staff/ therapy teams/ discharge hub/discharge team sister/charge nurse.

Gathering patient social information on admission, referring to discharge hub in a timely manner when patients are medically stable for discharge.

Liaising with family/friend carers to discuss any concerns they may have for discharge

Raising discharge concerns to the right teams as concerns arise, escalating delays in discharge to discharge team/site matrons.

Using clinical systems, e.g. ICE, Care flow connect, care flow live. Pro-track

Having a full understanding of all the Discharge pathways.

Gather accurate information of potential and planned discharges for the site matron to ensure flow throughout the trust

Attending ward board rounds daily

Excellent communication skills required for this role, be able to work within a team and on own initiative, friendly, approachable.
