Although this position is a 12 months fixed-term contract, the actual contract duration will be adjusted to account for the time taken to advertise the post, complete the hiring process, and finalise pre-employment checks. Barking, Havering and Redbridge have a growing ageing population, with around 18% now aged 65 or over. As this number rises, the need for proactive, coordinated care becomes more urgent. Older adults are at higher risk of frailty, long term conditions and complications from hospital admission, such as deconditioning, infections, delirium and loss of independence.
BHRUT aims to transform care by shifting from reactive crisis management to early identification and intervention. Central to this is a multi professional Comprehensive Geriatric Assessment, rapid same day or next day urgent appointments, and the ability to deliver short term IV therapies outside hospital to prevent avoidable admissions.
The Trust is building integrated pathways across acute, community and primary care and is recruiting skilled staff to deliver this vision. The specialist clinical pharmacist will play a key role in medicines optimisation, patient reviews, education, and service improvement within frailty services. This is an opportunity to help shape innovative, high quality care for the region's ageing population.
Main duties of the job
The post holder will be responsible for providing and developing comprehensive, patient orientated services for frailty across Barking Havering and Redbridge University Trust. They will work closely with the lead Pharmacist for Frailty and out of hospital care to further innovate and develop the frailty service with the aim of reducing attendances to the emergency department, reduce acute admissions into hospital and ensure the safe and effective use of medicines for our elderly population.
