The role involves working very closely with the Acute Frailty Virtual Ward multidisciplinary team (MDT) and is a pivotal role in ensuring all patients within the Frailty Virtual Ward receive the best possible care and service. This exciting post is available for a dynamic and experienced Advanced Clinical Practitioner (ACP) with an interest in the provision of care to patients suffering from co-morbidities."
Main duties of the job
This role is part of a cross organisational multi-disciplinary team whose purpose is to provide high level quality care to people within their normal place of residence as an alternative to a hospital inpatient episode. This role involves working in both Acute and Community settings and requires high levels of interoperability across organisations and interactions with many stakeholders and participation in the development of the virtual ward services and performance improvement initiatives.
The role of our Frailty Advanced Clinical Practitioners is pivotal to the delivery of this new pathway and model of care. Putting the frail person at the centre of our decision making and ensuring that we limit the risks of continued hospitalisation where we can 'add value' and proactively expediting discharge home for all.
Whilst working within the frailty team, the post holder will work in an autonomous role with a Frailty Consultant, reviewing frail older adults either within the Emergency Departments and Acute Frailty Units to identify patients suitable for admission to the virtual ward or working in the community setting visiting patients who have been admitted to the virtual ward monitoring their condition and ensuring their care plan continues to be appropriate.

These CDCs are open 12 hours a day, 7 days a week and are providing an extra 88,000 scans a year.
