Job summary

An exciting opportunity for a Critical Care Outreach Nurse has arisen in BHRUT. You will be part of a team providing care to deteriorating patients from both the wards and a busy emergency department presenting with a range of conditions requiring level 1 level 2 and level 3 care.

Our critical care units consist of a total of 57 beds across two sites - Queens and King Georges and you will be required to work across site on a rotational basis.

You will be a key member of the cardiac arrest team, attending all medical emergencies, and be able to act as a role model providing teaching and leadership support to the nursing and medical team.

You will work collaboratively within the multidisciplinary team to demonstrate high standards of individualised and evidenced based patient centred care.

You will be responsible for supporting changes based upon local and national guidelines and embedded into your care delivery will be the Trust PRIDE Values Passion, Responsibility, Innovation, Drive, and Empowerment.We are looking for a highly motivated, responsible person with a strong clinical acumen to be able to join our expanded Critical Care Outreach Team which runs accross site twenty four hours, seven days a week.

You will be a skilled and competent critical care nurse able to utilise clinical expertise, leadership skills, education and management to ensure a seamless service for patients, careers and staff, providing advice and support.

Main duties of the job

The post holder will be an integral member of the critical care services providing visible leadership within the critical care services.The post holder will deputies for the Matron for critical care outreach as required.

To provide expert advice, support and direction to ensure holistic patient care for acutely ill and deteriorating ward patients in conjunction with the multi-disciplinary teams and with the support from the matron.

  • Takes responsibility for triaging, co-ordinating and overseeing the organisation of patient care for acutely ill ward based patients on a day-to-day basis.
  • Be a credible practitioner who facilitates expert care to acutely ill or deteriorating patients in collaboration with other members of the multidisciplinary team.
  • Work closely with the intensive care nursing and medical teams to facilitate early and timely admission to ITU/HDU and safe discharge from critical care.
  • Receives referrals from all wards at in the Trust. Discusses care of any critically sick patients and advises on treatment within the agreed protocol. Summarizes all patient assessment in medical notes and advises staff of recommendation made.
  • Document clearly by using electronic and paper notes
  • Ensures that further referrals are made to other specialist teams where appropriate.
