Job summary
We have an exciting opportunity for a Discharge Co-Ordinator to join our friendly team on Sky A (Medical Short Stay Unit). We are looking to appoint an enthusiastic candidate with a friendly and helpful approach, and the confidence to effectively communicate with staff, patients, relatives, and external care providers (for example care homes).
The post holder will be working closely with the multidisciplinary team to plan, manage and ensure safe discharges of our patients who no longer meet the criteria to reside in an acute hospital setting. The post holder will be completing daily board rounds with the multidisciplinary team and will be responsible for advocating patients discharge needs.
The post holder will also be completing referral paperwork alongside the ward clinicians to ensure safe discharges which will include D2A and Fast Tracks. They will have to be able to manage a case load of patients and prioritise according to patient need to ensure a safe and supportive discharge from hospital.
The post holder will also need to work on their own initiative and be able to provide accurate discharge plans for the patients.
Main duties of the job
- Responsible for working as part of a team liaising with inter-disciplinary team members regarding the planning of discharge for patient's identification of potential discharge delays.
- Monitor patients predicted date of discharge and highlight any issues to ward staff if the patient may exceed this date.
- Working independently in order to facilitate discharges in a timely fashion and to highlight any concerns to the appropriate manager.
- Liaising with other agencies, patients, family and/or carers in order to provide effective communications to assist with timely discharge of patients.
- Liaise with the Junior Sister / Charge Nurse to seek advice/ assistance with any complex discharge issues.
- Attend daily board rounds to provide information and receive tasks
- Liaise with Nursing, Medical and Therapy staff and external agencies to ensure equipment, documentation or other aids are in place prior to discharge.
- Where appropriate liaise with Care Homes and coordinate a safe and effective discharge. Escorting the patient if appropriate

These CDCs are open 12 hours a day, 7 days a week and are providing an extra 88,000 scans a year.
