Job summary
We have an exciting role in the Geriatric Division for a Discharge Pathway Coordinator on Beech Frailty Unit. Care of the Elderly is currently going through a process of change to ensure we meet the on-going needs of our elderly patients and to provide an excellent standard of care every time and every day. This is an ideal opportunity for you to bring your experiences and visions to help us to deliver these changes
We are looking for an enthusiastic care pathway facilitator with post experience working in elderly care and has a passion for delivering safe discharge and reduce the length of stay of our elderly patient, putting the patients at the centre of our PRIDE values and promoting good patient experience.
You will work in a busy acute ED setting for frail and elderly patients with frailty score of 6 and above and will ensure that all complex discharges are dealt with in a timely manner within the agreed timescales and patients are discharged safely.
Main duties of the job
You will be required to establish and maintain effective working relationships with both Trust staff and relevant external agencies and advice the senior sisters and charge nurses /matrons/Lead Nurse, PALS and Family Liaison on areas of risk to the organisation and take necessary actions as appropriate to ensure that patients, relatives and carers who raise complaints regarding poor discharge/discharge alert are dealt within a sensitive, effective and confident manner in line with Trust discharge Policy.
You is required to have a proven experience working in an acute hospital clinical setting and must demonstrate good and sound knowledge in computer skills including Microsoft word and excel and Care Flow.
The individual will have excellent writing skills, communications skills and good telephone manners and has an experience dealing with the public
To attend the daily board rounds meetings and compile a list of all new discharges and completing D2A, Fastrack paperwork etc.
To Coordinate transfers to the exterior centre
To attend the weekly discharge planning meeting as instructed by the medical team
Liaise with the admission/bed manager for allocation of beds for inpatients, patients and Queens transfers from A&E or the observation ward
Attend the clinical ward rounds to be able to take instructions from the medical team and senior nurses to co-ordinate and facilitate discharge planning

These CDCs are open 12 hours a day, 7 days a week and are providing an extra 88,000 scans a year.
