Job summary

Saint Francis Hospice is seeking to recruit a doctor with a special interest in palliative medicine for a full time, fixed term Specialty Doctor post from 05.06.2024.

It presents a real opportunity for anyone considering a career in palliative medicine or wanting to develop a portfolio career with palliative medicine as a developed skill or special interest. Previous specialty doctors have a 100% success rate in moving on into their training placement of choice.

Conditions of service and salary are as per national medical and dental terms and conditions of service, with NHS contract held by Barking Havering and Redbridge University Hospitals NHS Trust (BHRUT).

Main duties of the job

This post would be ideal for a doctor seeking to gain hospice experience towards application to a Specialist Palliative Care training rotation or seeking to consolidate/expand their palliative care knowledge and skills. We are looking for a doctor who has completed their foundation years' training, and at least 2 further years of relevant clinical practice (i.e. eligible for Specialty Doctor status).

Our hospice is 4 miles from Romford station, in beautiful countryside on the London-Essex border. We have 18 specialist palliative care beds and a busy community service, which includes a 24/7 helpline, Community CNS, medical and wider MDT visiting service and Day Therapy services.

We see and support people with complex problems related to both advanced malignant and non-malignant conditions, frequently with multi morbidity.

We serve Outer East London (Redbridge, Barking & Dagenham, Havering), near West Essex and Brentwood. Barking, Havering and Redbridge University Hospitals NHS Trust is our local Trust. BHRUT comprises Queen's Hospital in Romford and King George Hospital in Ilford. We are a regional education centre experienced in supporting doctors in training.

The post holder will join a medical team of 3 Consultants, 3 Specialty Doctors and 2.9 GP trainees, and will provide middle grade support to the hospice inpatient unit, with opportunity for community sessions dependant on experience.

About us

London boroughs, and also the residents of near SW Essex (predominantly Brentwood).

BHRUT services include all the major specialties of large acute hospitals, operating from two main sites - King George Hospital in Goodmayes and Queen's Hospital in Romford. BHRUT has two of the busiest emergency departments in London. We are particularly proud of our regional Neurosciences Centre; Radiotherapy Centre; Hyper Acute Stroke Unit; and dedicated breast care service at King George Hospital. Saint Francis Hospice is situated in a beautiful rural location in Havering-atte-Bower, only 20 minutes' drive from Romford (and BHRUT Queens Hospital) and with excellent road links with London and Essex. About 1/3 of our inpatients are referred in from BHRUT; about 2/3 come in from the community. We are are a centre of excellence for specialist palliative care and achieved a CQC rating of 'outstanding' when last inspected.

Job responsibilities

CLINICAL

INPATIENT UNIT

  1. To share in the provision of medical care for the inpatients, working closely with other members of the medical team. This will include contributing to the planning of admissions, clerking in of new patients, review of patients, construction of a management plan, planning of discharges and facilitating of advance care planning, with a key duty to support our more junior GP trainees in their day-to-day practice on the ward, and to contribute to the support of relatives and carers.
  2. To attend and support daily ward handover meetings, provide specialty doctor led ward round inpatient review/support for the GP trainees who provide day to day care, and to participate in the Consultant Ward Round.
  3. To be a core part of the Inpatient Unit Multi-Professional Team Meetings, working closely with the rest of the multi-professional team to achieve the best of care for patients.
  4. To support good therapeutics practice, with a lead role in the weekly medicines review of all patients (the weekly therapeutics meeting)

COMMUNITY SUPPORT, IN THE OUTPATIENT UNIT AND AT HOME

  1. Dependant on experience, to share in supporting the Specialist Community Crisis and Support Team of CNSs and other clinical specialists who provide front line support to people at home

ON-CALL DUTIES

  1. On-calls consist of 1:4.5 weekends on-call (9am-5pm on site at standard rate, night cover non-resident low intensity), plus 1 in 5 weekday nights on call remunerated as standard rate until 7pm, then non-resident low intensity overnight.
  2. You will have Friday as a rest/lieu day preceding the weekend and the Tuesday following the weekend as a rest/lieu day.
  3. Must be able to attend the hospice within 1 hour during non-resident on-call periods.

IN SUPPORT OF THIS CLINICAL WORK

  1. To work in partnership with and liaise closely with our Referrals team, General Practitioners, relevant hospital teams and relevant community specialist teams, providing written communications when necessary.

TEACHING

  1. To support our junior (ST1, ST2) GP trainees with informal and bedside training/teaching and with tutorial teaching.
  2. To contribute to the educational programmes provided by the hospice for other health professionals.
  3. To support any healthcare professionals on placement at the hospice (e.g. Care of the Elderly clinicians, GP Registrars, medical students, nurses) to get the most out of their hospice visit

EDUCATIONAL/SERVICE IMPROVEMENT/AUDIT

  1. To take an active part in identifying areas for service improvement or audit, supporting service improvement projects and audits in progress. Mentorship of more junior staff to develop their knowledge of audits, and completion of audit ideas.
  2. To attend and contribute to relevant in-house educational meetings, including journal club.
