B7 Team Lead/Occupational Therapist (Community) 37.5 or 30 hours considered, based at Garswood Clinic, St Helens

The Cheshire and Merseyside Rehabilitation Network (CMRN) is a unique collaborative integrated pathway delivered in a hub, spoke and community model providing specialist rehabilitation to patients who have suffered traumatic injury or illness dependent on need and not diagnosis.

We require a suitably skilled and experienced Occupational Therapist to uptake role of Team Leader for our St Helens and Knowsley Community Specialist Rehabilitation Team, which provides specialist inter-disciplinary rehabilitation to patients with highly complex needs. As part of our wider community service, you will also need to work in close collaboration with our other community team which covers Liverpool and Sefton.

As an established community rehabilitation specialist, you will oversee and coordinate the teams work load, ensuring effective overall case-load management and patient flow. Adopting a flexible and adaptable approach, you will manage a clinical case-load, in addition to team lead duties.

Main duties of the job

The post holder will take a key role in the clinical leadership and day-to-day management of the Community Specialist Rehabilitation Service (CSRS) Team provided by the Cheshire and Merseyside Rehabilitation Network (CMRN), by providing clinical leadership to a designated locality team.

2. To oversee overall team clinical case-load through regular weekly meetings, ensuring timely completion of discharge reports.

3. To provide expert assessment of specialist rehabilitation needs and provide contribution to the clinical workload by maintaining a case-load of patients relative to own area of professional practice.

4. To identify areas of risk and contribute to the weekly multi-disciplinary MDT risk assessment, escalating concerns to the CSRS Lead as required.

5. To maintain effective working relationships with the Rehabilitation Hubs and Spoke Units staff, to support them in their role of discharge planning and optimising inpatient length of stay.

6. To attend the CMRN operational and clinical management meetings as required, and cascade business and/or work streams information to team and feedback information from the team to the CSRS Lead and relevant meetings and committees, ensuring agreed actions are followed through.

7. To ensure monthly submission of performance data and reports from patient administration system (PAS) and other clinical information systems.

8. In partnership with CSRS Lead, CMRN Manager and CMRN Performance, Quality and Education Manager to communicate any operational service changes and developments to key stakeholders, other health professionals, service users and the public as appropriate. To ensure that key staff, staff representatives, users and their representatives and the public are appropriately involved in any service development.

9. To ensure appropriate team attendance and contribution to relevant steering/networking groups relating to service area of work as requested.

10. To develop and maintain professional links with other Service/Professional/Community Clinical Leads across the sector and neighbouring organisations in order to share information, co-work and inform best practice.

11. To represent the CSRS Team within the network and the organisation and externally, to appropriate multi-agency groups/professionals and agencies as required.
