SMART stands for surgical and medical acute response team. This role is an exciting and continually evolving role, the individual will act within their professional boundaries and they will demonstrate safe clinical decision-making skills. The SMART nurse will be a clinically credible nurse who is proficient and experienced in looking after the critically ill and deteriorating patient. They will utilise their clinical expertise and specialist knowledge to deliver high quality evidence-based care to the critically ill patient. They will have a thorough understanding of what track, trigger, respond and escalate entails within the scope of the deteriorating patient. They will support the wider provision of a critical care outreach service assisting timely admissions as well as avoiding admissions to critical care, provide a follow-up service for all discharges from critical care and provide an ongoing plan of care for the patients in the ward setting. They will also form part of the cardiac arrest team and become an ALS provider.
Main duties of the job
On a daily basis the SMART nurse will review known patients and assess new patients in a time appropriate manner who are considered at risk by the MDT who express a clinical concern or those patients who trigger via the use of NEWS2.
The SMART nurse will ensure all emergency admissions into the Centre are seen triaged and clerked within 30minutes ensuring that admission documentation and clinical management plans are implemented in a timely manner.
The SMART nurse will have the exciting opportunity to support and develop various new services such as supporting a vascular access and insertion service and the thrombectomy service. The thrombectomy service will involve providing clinical support, recovery and maintaining the safety of the patient along the patient pathway acting as the clinical advocate in the delivery of the care that the patient requires.
They will be required to work flexibly as a member of SMART to ensure standards are being maintained, evaluated and where necessary improved.
The SMART role provides and prides itself on support and education given to other members of the MDT and the post holder will be expected to act as a role model and participate and take a lead on providing education to the wider MDT.
To be competent in tracheostomy care and management and work along side and support the MDT tracheostomy ward round.

They will also be expected to take the lead on developing themselves with support of their manager in line with the role and continued professional development to support the service provision.
The role covers a 24hour service 7 days a week by rotating onto days and nights and the willingness to be flexible is essential to ensure the service can be delivered.
