Band 7 Frailty Clinical Nurse Specialist

We are delighted to be offering an exciting opportunity to recruit to the Clinical Nurse Specialist role for Frailty and Older People within the Urgent, Emergency and Elder care group (UEE).

This is a senior band 7 post working at the front door with the Acute Care of the Elderly medical teams and front door therapy teams to holistically assess the older frail patient using their knowledge of Clinical Frailty Scoring and acute care needs to ensure the correct clinical pathways are applied to achieve the best outcomes for this patient group.

You must be passionate about older peoples care and demonstrate a commitment to ensuring a positive journey is achieved by using your specialist knowledge and skills to establish the correct pathway either back out to the community or within acute care where indicated. Avoiding hospital admissions and reducing length of stays is an integral part of this role and your specialist assessment and follow up of the patient's care will demonstrate the key performance indicators of this role.

You will be expected to work both autonomously and within a team and achieve your Non-medical Nurse prescriber qualification for which you can expect to be fully supported. This role will form part of the foundation for a further career pathway in older people's services as the care group explore Advanced nurse practitioner nursing roles for the future.

Main duties of the job

Working within the Integrated Services for Older Peoples and Acute care of the Elderly (ISOP/ACE) Team, you will be responsible for delivering effective comprehensive assessment and management of a designated cohort of complex frail and vulnerable older adults in our acute care services (specifically in the Emergency Department and on the Older Peoples Assessment and Liaison and Frailty Units.

To support with promoting and monitoring the Trusts front-door frailty screening system for unscheduled admissions

To contribute and develop skills in taking a relevant history and clinical assessment and develop a management plan as well as evaluate the progress of that plan within the limitations of their knowledge and skills while working with other members of the CNS for older people, medical and other members of the multi-disciplinary team (MDT).

They will demonstrate clinical judgement and critical decision-making skills based upon established guidelines to co-ordinate and communicate a Comprehensive Geriatric Assessment (CGA) and its outcomes, informing compilation of problem lists and care/treatment action plans.

Working collaboratively with hospital and community health and social care partners to provide personalised patient-centred care/treatment plans.

Using an MDT approach and specialist assessment skills identify patients suitable for transfer to the Community Assessment and Treatments Units (CATU) as part of the frailty pathway - facilitate this transfer by providing handover to both medical and nursing colleagues

Working as part of the CNS for older people team and with front-door frailty care colleagues to deliver performance targets focused on promoting attendance and admission avoidance (whenever appropriate and in the older persons best interest)

Sustainability is integral to the Trust achieving the NHS Net Zero target. All staff are therefore actively encouraged and supported to implement new ways of working within their field of expertise.
