Job summary
An exciting opportunity has arisen to work within the Cardiac Specialist Nursing Team, as a Cardiac Rehabilitation and Surgical Liaison Nurse Specialist at The Royal Cornwall Hospitals NHS Trust.
This role is a part time 22.5 hours per week position. You will be based at the Royal Cornwall Hospital.

Main duties of the job
The post holder will be a member of a small team responsible for providing the following key services:
Cardiac Rehabilitation (CR) Inpatient Service (Phase 1 and 2 CR)
Providing all suitable cardiac patients with the pre-hospital discharge component of the CR service (Phase 1). Delivering information relating to diagnosis (including investigation results), and education about risk factors, lifestyle modification, chest pain management, driving advice, returning to work advice, medications and exercise advice. Providing psychological support and coaching and where required referral / signposting onto other services.Service delivery is in line with the British Association of Cardiac Rehabilitation and Prevention (BACPR) and NICE recommendations.
Delivery of the Phase 2 cardiac rehab clinic (following discharge from hospital) and referral onto the community run Cardiac Rehabilitation exercise programme if appropriate.
Cardiac Surgery Liaison Service
Providing support and education for cardiac inpatients and their families at the point of acceptance for surgery, through to transfer. Co-ordinating the cardiac surgical pathway by liaising with the surgical centres and the team at RCHT. Facilitating and organising the relevant surgical work up in a timely manner.
Cardiac Rehabilitation Exercise Programme for Truro area
Supporting the Truro group exercise and education programme in line with the BACPR standards.
