Job summary

The Virtual Ward initiative is a new opportunity for Recovery at Home who currently provide Urgent Community Response to increase their capacity to support patients to receive hospital at home care across Sunderland and South Tyneside. The teams are based in Clarendon in Hebburn and Lechmere in Sunderland.

The team consist of an extensive MDT which includes nurse practitioners, nurses, a general practitioner, occupational therapists, physiotherapists, health care assistants, therapy assistant/exercise practitioners and administrative staff. Our virtual ward is overseen by a Consultant. The team also works closely alongside partner agencies including those from social care, assistive technology and voluntary sector services.

This role would involve assessing people in their own homes who are either acutely unwell or have recently been discharged from hospital and require enhanced intervention and short term support for their physical and functional health needs to remain at home.

The service is fast paced and you would be expected to see a wide range of patients who present with a variety of conditions in their own homes and or virtually. Our virtual ward pathways cover respiratory conditions and frailty.

This is a unique and rewarding role which will allow you to work with patients within their usual place of residence to enable them to remain there safely alongside you developing your own clinical skills working as part of a dedicated MDT.

Main duties of the job

The Recovery at Home are nurse led service based in Sunderland and South Tyneside, which delivers rapid assessment and treatment to patients in their own home with an acute illness and/or injury. The Team provide holistic care/intervention 24/7, with the intent to prevent admission/readmission to hospital. You must be dynamic, motivated and enthusiastic with a flexible approach to working with relevant experience in an acute care environment.

You will work within Directorate One of Community Services, supported by senior colleagues who are passionate and proud of the services we deliver to our community of South Tyneside and Sunderland. The service is committed to developing new ways of working with partner agencies and the wider healthcare economy supporting people in their own homes. The Team has a strong emphasis on continuous development and learning. You will need to have proven leadership qualities and act as a resource to junior colleagues.

You will be expected to identify and undertake a Service Improvement project. The post holder will need to be proactive with their own learning and work to identified timescales

The team cover 24 hours a day 7 days a week, there is an opportunity to work your full time hours, if applicable, over 3 longer shifts.
