Job summary

An exciting opportunity for a Respiratory Nurse Specialist to join the recovery at home team as part of the virtual ward initiative.

The Virtual Ward initiative is a new opportunity for Recovery at Home who currently provide Urgent Community Response to increase their capacity to support patients to receive hospital at home care across Sunderland and South Tyneside. The teams are based in Clarendon in Hebburn and Lechmere in Sunderland.

The team consist of an extensive MDT which includes nurse practitioners, nurses, a general practitioner, occupational therapists, physiotherapists, health care assistants, therapy assistant/exercise practitioners and administrative staff. Our virtual ward is overseen by a Consultant. The team also works closely alongside partner agencies including those from social care, assistive technology and voluntary sector services.

This role would involve clinical assessment and care planning for patients with respiratory conditions. You will be responsible for care planning, prescribing and advising patients how to maintain their health at home.

Respiratory MDT pathways are being developed where support for patients in their own home will be provided.

Opportunities for training and development will be discussed at yearly appraisals.

Main duties of the job

The service is fast paced and you would be expected to see a wide range of patients who present with a variety of conditions in their own homes and or virtually. Our virtual ward pathways cover respiratory conditions and frailty.

This is a unique and rewarding role which will allow you to work with patients within their usual place of residence to enable them to remain there safely alongside you developing your own clinical skills working as part of a dedicated MDT.

As a skilled respiratory Specialist nurse we would look to you being central to patients with respiratory conditions.

You will be part of a MDT, with a supportive approach to help a patient remain in their own home.

Clinical experience and skills linked to respiratory nursing are essential.

You will need to have great communication skills and be experienced in decision making and being autonomous.

Your role will cover 24 hours a day and 7 days a week, there will be opportunities to discuss working patterns.

Main Purpose of the Job:

The post holder will work as a respiratory clinical nurse specialist for the Recovery at Home Team, based across the Trust locations when needed.

The post holder will need to work autonomusy and as part of a multidisciplinary team delivering Virtual Ward within the community setting. They will use a a high level of clinical and diagnostic reasonsing skills that will enable a holistic patient assessment for acute conditions, enabling patients to be safely cared for at home using the Virtual Ward philosophy. They will act as a expert in the field of respiratory nursing for other members of the multidisciplinary team: providing education, advice and support to ensure an evidence based approach to safe decision making and evidenced based care, using agreed Trust pathways.

This role will include working across the wider health system to ensure the delivery of proactive management of this group of patients, implementing and reviewing clinical management plans to support patients to remain safely at home, and prevent any unplanned hospital admissions, using caseload management skills.

To lead a continuously improving service from a respiratory pathway perspective, by ensuring systems are in place to support clinical governance, quality monitoring and ongoing research and audit of practice pathways for patients on the Virtual Ward. Adopting local and national best practice and evidence based care to continually enhance the service.

The role may include caring for patients and relatives who are anxious, confused or distressed and clear communication skills and empathy is required to offer reassuring and guidance. A part of each working day involves talking to patients either via the telephone or face to face

Provide a high-quality support service to patients on the Virtual Ward caseload.

If required, will manage staff including assuring completion of supervision, appraisals and compliance with mandatory training

Oversee and delegate remote monitoring of patients for the VW respiratory pathway.

Undertake clinical assessment of patients on the Virtual Ward and develop, implement and evaluate care and treatment plans. Use their advanced assessment skills to assess patients who are on the virtual ward, and whose observations taken via remote monitoring require further assessment and review face to face.

Active case management will involve early identification of patients whose condition is at risk of becoming unstable or who needs increased monitoring by the Virtual Ward Team.
