Job summary

An exciting opportunity has arisen for an enthusiastic, well-motivated and flexible band 7 Physiotherapist to join our Virtual Ward and Urgent Community Response Service in Sunderland and South Tyneside.

The team consists of an extensive MDT which includes nurse practitioners, nurses, occupational therapists, physiotherapists, health care assistants, therapy assistant/exercise practitioners and administrative staff. The team also works closely alongside partner agencies including those from the local and voluntary sector.

This role would involve assessing people in their own homes who are either acutely unwell or have recently been discharged from hospital and require enhanced intervention and short term support for their physical and functional health needs to remain at home.

The service is fast paced and you would be expected to see a wide range of patients who present with a variety of conditions.

This is a unique and rewarding role which will allow you to work with patients within their usual place of residence to enable them to remain there safely alongside you developing your own clinical skills.

Main duties of the job

We are looking for a skilled and initiative physiotherapist who can be involved in a new development.

Being flexible and dynamic is a skill we require, who can work with the patients and multi professional team, building the service to support patients in their own home.

You will be working autonomously, assessing individual patient needs, initiating investigations, determining a differential diagnosis and initiating appropriate holistic, evidence based treatment and care. Refer patients who require medical intervention to the appropriate services.

The service delivers 24 hour care over 7 days.

You will be supported in the team with a yearly appraisal and a development plan.
