Part Time post-30 hours per week
An exciting opportunity has arisen for a band 4, Alcohol Care Team
Practitioner to join the Alcohol Care Team (ACT) at STSFT. ACTs
are teams of alcohol specialist clinicians, based in acute hospitals which
provide specialist support, predominantly to alcohol-dependent patients.
The post will require flexibility, working across Sunderland Royal and
South Tyneside Hospital, you will also contribute to covering a 7 day
service.
The successful candidate will be able to demonstrate excellent
communication skills, team working skills and will be dedicated to improving
the patients' experience. You will need to demonstrate continuing professional
development, some experience working with this patient group and ability to work
closely within a multi-disciplinary team.

Main duties of the job
The Alcohol Practitioner will be an
integral member of the Alcohol Care Team, which provides care and treatment for
patients with alcohol misuse issues in a hospital environment. Working closely
with the Alcohol Care Team, the Alcohol Practitioner will assess the needs of
the individual patient and deliver appropriate psychosocial interventions and
risk assessments.
Provide training to employees on
screening and brief interventions and support the Alcohol Care Team in developing
and delivering education to Trust employees in the speciality of alcohol misuse
Responsible for managing a caseload
of individual patients who are not engaging in the community but continue to
come to hospital to reduce alcohol health harms through a wide range of support
and interventions.
Flexible to meet the needs of the
team and will demonstrate knowledge, expertise and passion in working within
the speciality of alcohol misuse
To develop therapeutic relationships with clients and
family/carers.
To have knowledge of the impact excessive
alcohol consumption has on physical health, mental health and social issues and
to use this knowledge to fully assess patients
To
provide brief interventions, psychosocial interventions and motivational
interviewing as appropriate
To screen patients to identify those at risk
from alcohol related health and social problems and those whose health is being
damaged by alcohol
