An exciting
opportunity has arisen in the Renal Department at Sunderland Royal Hospital.
The Living
Kidney Donor Nurse (LKDN) Specialist would like to invite applications from
enthusiastic motivated Registered Nurses with an interest in Renal and/or Organ
Donation.
The LKDN
Service operates across South Tyneside and Sunderland and is based in
Sunderland Royal Hospital
The successful
candidate will receive full training and be supported by the existing LKDN
Specialist, a willingness to attend study days and courses off site is
necessary due to the training needs of the role
Main duties of the job
Applications
are welcome from Registered Nurses with excellent clinical skills and the
ability to communicate well on a professional level within the MDT and the
delivery of sensitive and confidential information to other health care
professionals and service users
The successful
candidate will be expected to use good IT and word processing skills for
patient notes and clinic information

To co-ordinate the delivery and
development of the Renal Transplant Service using effective evidence based
practice in line with agreed national standards for patient safety and
monitoring;
To lead Specialist Nurse led
clinics and be responsible for planning, co-ordination, delivery and
evaluation of nurse led clinics;
To be accountable for the planning
and delivery of all aspects of patient care in relation to Renal
Transplantation;
To liaise with all key members of
the multi-disciplinary team in the delivery of specialist patient care;
To participate fully in the
multi-disciplinary team meetings and work with the Consultant Nephrologist
and Surgeons to deliver specialist clinical care appropriate to the needs
of the patient;
To continuously improve clinical
care standards in line with the clinical governance framework;
Demonstrate leadership in
contributing to a culture of patient safety and continuous improvement in
line with the clinical governance framework;
Be accountable for provision of a
specialist nursing service, using clinical skills in the assessment,
diagnosis, prescribing and delivery of treatment plans to support all
aspects of patient care across the pathway;
Co-ordinate the provision of
services to patients within specialist area, ensuring continuity of care
across the Trust and between primary and secondary care;
Play a key role in
facilitating hospital discharge and preventing avoidable hospital admissions
As
an expert practitioner, be accessible to provide education and training
and clinical advice and support to Trust staff, primary care staff,
patients and relatives/carers;
Participate in / lead on audit and
research relevant to the needs of specialist area;
Participate in service development
within renal services across the Trust;
Contribute to the achievement of
national and local standards/targets;
Support the Matron or Team Lead to
facilitate co-ordination and delivery of a service which is both efficient
and evidence-based.
