We are looking for a caring, motivated and enthusiastic Band 3 Healthcare Assistant to join our dynamic Heart Failure Service. This is an exciting opportunity to play a key role in supporting patients living with heart failure across our community and hospital settings.

As part of a dedicated multidisciplinary team, you will work closely with specialist heart failure nurses, cardiologists and other healthcare professionals to deliver high-quality, patient-centred care. Your role will involve supporting clinical assessments, monitoring patients vital signs and symptoms, assisting with education and self-management plans, and contributing to the smooth delivery of the heart failure pathway.

This is a rewarding position where you can make a real difference to patients lives, helping them manage their condition, improve their quality of life, and reduce hospital admissions. Full training and ongoing support will be provided to help you develop your skills within this specialist area of cardiac care.

Main duties of the job

STSFT Integrated heart failure service aims to provide exceptional patient care inline with the recently developed heart failure gold standard framework. The role will include the following duties.

Support the heart failure nursing team in the assessment, monitoring and follow-up of patients in clinic, community and virtual settings.

Record and report clinical observations accurately, including blood pressure, pulse, weight, oxygen saturation ECGs and take bloods.

Provide basic education and support to patients and carers on heart failure management, medication adherence, and lifestyle advice.

Assist with data entry, audit, and maintenance of accurate patient records.

Contribute to service improvement initiatives and help ensure the highest standards of care are maintained.

Work in accordance with Trust policies, procedures, and infection control guidelines

  • Presents self as courteous, respectful and helpful.
  • Basic keyboard and IT skills.
  • Willingness to complete Competency Based Assessments and a Portfolio of evidence, in order to achieve the Care Certificate and any other training related to this role.
  • Work effectively under direct supervision as part of a multi-disciplinary team.
