Job summary

Part Time: 20 hours

We are looking to recruit an experienced senior nurse to work as a Band 7 Parkinsons Disease (PD) Specialist Nurse within the Neurology Specialist Nursing service at South Tyneside and Sunderland NHS Foundation Trust.

We are seeking a knowledgeable individual with highly developed interpersonal and excellent communication skills and has a passion for delivering excellence in care and driving improvements and developments across the PD service which sit within the Rehab and Elderly Medicine Directorate.

Successful applicants will need to demonstrate an interest and sound clinical knowledge as well as an understanding of the specialist care and management required by patients across the PD pathway. Whilst the specialist nurse role provides inpatient, outpatient, and an in-reach service to support patients, carers and staff with specialist advice, education and training in managing PD and its associated complications. We welcome applications from experienced or existing Band 7 PD specialist nurses, as well as nurses working at Band 6 level aspiring to progress into a specialist nursing role within neurology.

Main duties of the job

Main Duties of the post holder will include:

Clinical

Ensure provision of expert specialist advice and knowledge to patients, carers, and staff regarding Parkinsons Disease (PD)

Accountable for the planning and delivery of patient care in relation to the PD service

Pro-actively manage own workload and clinical caseload of patients, making decisions regarding care, treatment and support required.

Liaise with Neurology consultants, co-ordinate the delivery of the PD patient pathways through the use of effective evidence based practice and national standards for patient safety and monitoring

Manage/lead specialist nurse led clinics and playing a key role in optimising complex treatment regimes, supporting hospital discharge and preventing avoidable hospital admissions

Managerial

Attend meetings regarding PD care and represent STSFT as an organisation

Participate in service meetings, neurology CG and directorate level discussions

Contribute to MDT meetings ensuring patients are appropriately reviewed and treatment options discussed

To represent the service and play a key role in ensuring that national and local standards/targets are met

Audit / Research / Governance

Participate in audit, research and governance activity relevant to the PD service

Continuously improve clinical care standards in line with the STSFT CG framework
