An exciting opportunity has arisen at STSFT for an experienced leader who is keen to develop their managerial portfolio and join our experienced clinical site management team. The Clinical Site Manager is key to ensuring that services continue to operate in a clinically effective and safe manner both in and out of hours. Out of hours being the senior individual on duty, whose responsibility it is to provide clinical oversight and assurance across STSFTand be the first point of contact for wards/departments requiring support/guidance and managing patient flow throughout the hospital. You will be instrumental in maintaining and delivering high standards of quality care and patient safety.

Main duties of the job

The role requires the individual to provide strong clinical leaderships, be highly visible and accessible to all staff groups. There is requirement to ensure that appropriate systems and processes are in place providing continuity of safe and effective care, supporting a safety culture within the organisation. The role ensures that there is an optimum use of resources to accommodate patients in a clinical area, which most accurately reflects their presenting condition. The role requires day to day operational responsibility and accountability for patient flow across all Divisions and is pivotal to the organisation sustaining flow throughout, while evaluating escalation status both internal and external.

  • Lead by example to drive up standards of clinical care and improve patient outcomes, through promoting teamwork, and fostering a positive and accountable environment.
  • To actively manage the emergency and elective admission process, across the site, to provide safe and appropriate patient placement and achievement of related trust targets whilst maintaining contingency plans for emerging leads.

Responsible for ensuring an accurate reflection of capacity and demand is always available to support senior leaders to assess patient safety and overall trust capacity.

The post holder will act as senior clinical decision maker to ensure all decisions affecting patient flow supports patient safety and quality throughout their journey from admission, through to timely and safe discharge.

Crucial role in supporting and collaborating with the emergency department in timely ambulance handovers by ensuring efficient processes, effective communication, and clear escalation pathways within the hospital setting. Minimising delays, improving patient flow, and optimise the overall performance of the emergency pathway.

The role focuses on ensuring compliance with national standards with a responsibility for proactively preventing breaches, such as the 4-hour, 8-hour and 12-hour standards. While assisting the validating of breaches that have occurred against these standards.

Provide a senior leadership role within Emergency department Huddles and touch points, managing surge, volume and capacity to create patient flow.

Provide professional challenge and support to ensure optimal flow and safety of the Emergency Department and throughout the organisation.

Responsible for providing information and action plans for the cancellation of non-urgent elective admissions in liaison with Theatres, Consultants and Ward Managers (in hours and out of hours).

Progress and chase outstanding actions from Site Meetings and within the Team which have been escalated from Directorates to enable patients to be transferred to the identified clinical speciality/pathway.

Critical role in facilitating the efficient movement of patients within the hospital setting and arranging repatriation of patients to and from the organisation within 24-hour timeframe.

Out of hours; to be accountable of the overall management of the hospital.

Out of hours; responsible for dealing with complaints from staff, patients and relatives.

Out of hours; responsible for being the first line contact in the event of a fire alarm, fire evacuation, major incident or any other emergency affecting the trust.

Out of hours; responsible for dealing and assisting with nurse staffing and clinical issues, giving advice, leadership and support as a senior nurse.

  • Ensure Patient Flow and patient safety is always maintained throughout the organisation.
  • Obtain an accurate bed state utilising the electronic tools whilst being visible at ward and department level to inform decisions regarding flow and bed capacity.

To recognise the importance of maintaining effective patient flow throughout the organisation, educating staff in this area exploring all avenues of improvement to constantly improve the experience for patients.

Have a detailed working knowledge of wards, admission pathways, elective flows and ward turnover to support decision making regarding overall site risks

Support ED coordinator and EAU coordinator in making correct flow decisions to ensure safety across their own departments. Escalate any concerns and barriers to flow which have the potential to cause flow or patient safety issues.

To attend all cardiac/fire/security alerts acting in a leading or coordinating role in support of the main team

Being responsible officer for fire team stand down in the event of activation. Act as Designated Nursing Officer/Designated Medical Officer for medical gases

Bed Management To maintain an accurate and contemporaneous bed state, always monitoring actual / potential occupancy, and to actively contribute to the required daily collection and analysis of data relating to bed management, providing commentary on expected positions throughout the day/night.
