As part of STSFTs continued commitment to improving the safety of patients and the effective utilisation of resources, we have an exciting opportunity for a patient-focused Advanced Practitioner Pharmacist to lead and contribute to delivery of the Renal Repatriation Service, and Critical Care Medicines Management Service. This is an excellent opportunity to develop and contribute to advanced-level, patient-focused service delivery within these Teams. You will be a key member of both multidisciplinary teams committed to delivering the highest standards of care.
South Tyneside and Sunderland NHS Foundation Trust has invested in modern pharmaceutical facilities including full electronic prescribing with decision support, automated dispensaries and extensive seven-day clinical services to enhance the patients experience of care by improving the quality, safety and effectiveness of their medicines utilisation. As part of this investment our nationally-recognised pharmacist-led Integrated Medicines Management Scheme has been expanded to all acute areas. Your role will be to lead and co-ordinate the professional clinical and pharmaceutical input for this patient.
Main duties of the job
You will be a registered pharmacist with post-registration experience in clinical services and well-developed clinical pharmacy skills. You will be able to provide evidence of effective service delivery at a specialist pharmacist level, post-graduate study to Diploma level and well-developed clinical pharmacy skills with an independent prescribing qualification and evidence of advanced clinical skills development. The ability to support and mentor junior pharmacists and undertake clinical supervision will be key skills.
The Pharmacy Service operates a seven-day clinical service. As a skilled and motivated Pharmacist with experience of delivering clinical pharmacy services, you will:
- Lead and co-ordinate the professional, clinical and pharmaceutical input into the Renal Repatriation Service and clinically-manage patients along with consultant physicians. You will work closely with the clinical teams and become involved with the ongoing clinical management of patients including attendance of the MDT Transplant Clinic and provision of medicines advice to patients via the patient help-line.
Deliver a highly-specialised medicines management service to both Adult Critical Care units in the Trust. Working closely with the clinical teams to contribute to the clinical review of medicines prescribed to ensure the cost-effective use. There will also be the opportunity to contribute to development of a post-ICCU step-down service, post-ICCU clinics and guideline development.
