Job summary
An exciting 12 month secondment opportunity has arisen for an
experienced clinical pharmacist to join South Tyneside and Sunderland NHS
Foundation Trust as a patient focused Advanced Practitioner Pharmacist working
in the Lipid service.
The role involves providing
patient focused advanced clinical support to the Lipid Service team and you
will work closely with the team to ensure a high quality delivery of service
for patients attending the Trust. You will have the opportunity to utilise
independent prescribing skills through the delivery of established pharmacist
led Lipid clinics, and work with the team to develop services within this
speciality.
Main duties of the job
You
will have evidence of effective service delivery at a specialist pharmacist
level, post-graduate study to diploma level, well-developed clinical pharmacy
skills with an independent prescribing qualification and evidence of advanced
clinical skills development. The Pharmacy Service operates a seven day
clinical service with routine support of both junior pharmacists and integrated
medicines management technicians.

full electronic prescribing with decision support, automated dispensaries and
extensive seven day clinical services to enhance the patients experience of
care by improving the quality, safety and effectiveness of their medicines
utilisation. As part of this investment our nationally recognised
pharmacist-led Integrated Medicines Management Scheme has been expanded to all
acute areas. Your role will be to lead and co-ordinate the professional
clinical and pharmaceutical input for this patient group with a focus on
medicines optimisation, safe and secure utilisation of medicines and minimising
the risk of inappropriate use.
