Job summary

Due to maternity leave, we have an exciting 12 month secondment opportunity for a patient focused advanced pharmacist practitioner in the Frailty Team. This is an opportunity for an advanced pharmacist practitioner to develop advanced level service delivery within the Frailty Team, support our existing team of admissions and frailty advanced practitioners working in multidisciplinary collaborative teams and further build on the delivery of our integrated medicines management to improve drug therapy and patient safety in high risk areas. Hence you will be expected to work closely supporting Directorate Lead Pharmacists in your specialty supporting the wider pharmacy advanced practitioner team across 7 days including delivering care from admission, through any inpatient stay and beyond into primary care.

Main duties of the job

You will have evidence of effective service delivery at a specialist pharmacist level, post-graduate study to diploma level and well-developed clinical pharmacy skills in an acute medical or surgical specialty with an independent prescribing qualification and evidence of advanced clinical skills. This post is part of a 5 day in 7 day rota of nine practitioners with routine support of both junior pharmacists and medicines management technicians.

South Tyneside and Sunderland NHS Foundation Trust has invested in modern pharmaceutical facilities including full electronic prescribing with decision support, automated dispensaries and extensive 7 day clinical services to enhance the patients experience of care by improving the quality, safety and effectiveness of their medicines utilisation. As part of this investment our nationally recognised pharmacist-led Medicines Management Scheme has been expanded to all acute areas.

To lead and co-ordinate the professional clinical and pharmaceutical input into the safe and cost effective management and development of the utilisation of medication in conjunction with Trust emergency care and re-admissions service and lead consultants ensuring their safe and secure use and minimising the risk of inappropriate use.

To provide expert advice in the designated area to all directorate undertaking risk management, financial management and clinical governance activities and undertake prescribing to deliver medicines optimization and safe and effective discharge

To monitor performance of pharmacists working in this area, to undertake clinical supervision and internal and external teaching for all staff

To co-ordinate clinical service delivery within the safe and sustainable care project focused on front of house and ambulatory care pathways.

To link with antibiotic lead pharmacist to deliver effective antibiotic utilisation and initiation to reduce the incidence of Hospital Acquired Infections.

To work at an advanced level in the designated areas of practice.
