An exciting part-time (22.5 hours per week), fixed-term / **secondment opportunity has arisen (until 31.05.26) for a clinical pharmacist to join South Tyneside and Sunderland NHS Foundation Trust as a patient-focused Advanced Practitioner Pharmacist working as part of the Frailty Team and the Dementia and Delerium Outreach Team (DDOT).
The role involves providing patient-focused advanced clinical support to the Frailty and DDOT Teams and you will work as part of the multidisciplinary teams to contribute to the management of Frailty and Dementia and Delirium patients to ensure a high-quality delivery of service for patients attending the Trust. You will have the opportunity to utilise independent prescribing skills through the attendance of Front of House consultant-led ward rounds and the delivery of pharmacist-led Dementia clinics, and work with the Teams to develop advanced service delivery within these specialties to support our existing team of Advanced Pharmacist Practitioners.
You will be expected to work closely supporting Directorate Lead Pharmacists in your specialty, supporting the wider pharmacy advanced practitioner team across seven days including delivering care from admission, through any inpatient stay and beyond into primary care.

Main duties of the job
You will have significant hospital clinical pharmacist experience with evidence of effective service delivery at a specialist pharmacist level, post-graduate study to Diploma level, well-developed clinical pharmacy skills with an independent prescribing qualification and evidence of advanced clinical skills development. You will have experience of working as part of a team, excellent interpersonal skills, a strong desire to learn and a meticulous attention to detail.
