Job summary

There is an opportunity to become part of a progressive clinical Pharmacy team, providing the professional, technical and logistical components of an Integrated Medicine Management (IMM) Service at South Tyneside and Sunderland Foundation Trust. These full and part-time Posts will be based at either South Tyneside District Hospital or Sunderland Royal Hospital but will cross-cover, as and when needed. We are looking for a Pharmacy Technician to join the IMM team. Working outside the Dispensary with dedicated Pharmacists at ward level you will provide a patient-focused service which includes the assessment of patients own drugs, determining and maintaining supplies of medicines to be stored at the patients bedside to ensure a safe and timely discharge service from the Hospital. The role will also involvecounselling of patients on their medicines to deliver medicines optimisation and completing medicines reconciliation. Main duties of the job

You must possess the relevant Qualifications and be a registered Pharmacy Technician. You will be appointed to a Band 4 Post until you have met the full Criteria to progress to the Band 5 IMM Pharmacy Technician role. This will include a minimum of two cycles of CPD revalidation as a registered Pharmacy Technician with the General Pharmaceutical Society. Completed accredited accuracy checking and the following Medicines Management Frameworks

Module 1 - Assessment of patients own drugs;

Module 2 - Ordering medication for one-stop dispensing;

Module 3 - Medicines Reconciliation.

After meeting the full requirements of the Band 5 role, including the above, you will progress into a Band 5 Post. This Post has a requirement for seven-day working including weekends, late nights, bank holidays and cross-site cover. You must be adaptable, have good communication skills and possess the ability to work on your own initiative to deliver patient care within the wider multi-disciplinary team.
