We
are recruiting registered nurses to our Long-Term Conditions Internship
Programme, due to start January 2026. The aim is to develop the specialist workforce
and provide excellence in the management of patients with long term conditions.
The LTC
Internship Programme aims to:
Support
succession planning within the specialist nursing workforce by providing an
opportunity for registered nurses to develop more specialist knowledge, skill
and experience through working directly with our clinical teams.
Retain
experienced and motivated qualified staff by providing career development
opportunities for individuals interested in the management of long-term
conditions and capable of progressing to specialist practice.
Develop the
clinical competence, skills and knowledge of each successful intern, to enrich patient experience in our clinical areas and
ensure excellence in patient care.
Enable
the intern through the programme learning outcomes to make positive service
improvements within their area of practice.
Main duties of the job
The
duration of the programme is 18 months. Successful applicants will be required to commit to 7.5 hours per week learning and be expected to:
Develop their
knowledge and skills through the utilisation of a competency framework.
Work as part
of a specialist team at STSFT, through the provision of clinical placements.
Interns will be placed within Rehabilitation and Elderly Medicine,
Cardiothoracic, and Diabetes.
Work
collaboratively with our acute and community teams, to increase the interns
understanding of the wider support needs and services involved in the patients
journey.
Complete -Chronic Disease Management for Advanced
Practitioners at level 7.
Complete -Clinical Skills training at level 7.
Potential
Leadership and Quality Improvement module at level 7 to be completed in 2027

- Mentorship certificate/Practice Assessor training or demonstrable equivalent experience
