and forwardthinking practitioner
to lead strategic improvements in the care of people living with incurable
cancer across acute hospital services and the community.
As the Living with Cancer Lead Practitioner (Band 8a), you will
play a key role in shaping how the Trust delivers timely, proactive and
personalised care at critical points in the patient journey. You will provide
senior clinical and strategic leadership, supporting services to respond
consistently and effectively to complex cancerrelated needs.
Working closely with acute specialties, Operational Cancer Teams,
oncology, palliative care, AHPs, emergency and urgent care services, and
community partners, you will help strengthen integrated pathways and promote
smooth, compassionate transitions of care.

Main duties of the job
The
postholder will lead the redesign and improvement of acute and community
pathways for people living with incurable cancer, with a focus on anticipatory
care and reducing avoidable hospital attendances and admissions.
They
will provide senior clinical leadership for the 24month Living with Cancer programme,
using structured project and improvement approaches to deliver agreed
milestones, KPIs and measurable outcomes.
Key
responsibilities include:
Working
collaboratively across acute specialties, oncology, palliative care, AHPs,
Operational Cancer Teams, emergency and urgent care services and community
partners to strengthen coordination of care.
Providing
expert clinical advice, coaching and education to support staff managing
complex cancer presentations
Identifying
workforce development needs and delivering targeted improvement actions.
Designing
and embedding clear referral, handover and transition processes.
Using
data, audit, patient insight and quality intelligence to evaluate impact and
reduce variation
Reporting
progress through governance forums; and contributing to learning from
incidents, complaints and service review to inform future service development.

- Registered Clinical Professional - Current NMC Registration (Level 1 adult nurse) / Current HCPC qualification.
- Degree in nursing studies or relevant clinical degree.
- Masters level study, or equivalent specialist study / experience.
- Mentorship/teaching qualification or equivalent and/or Practice Assessor training.
- Team management experience (staff management, change management, project management, budget management).
