Job summary

South Tyneside and Sunderland Foundation trust are looking to appoint an Acute Learning Disabilities and Autism Team Manager, youll lead a team that ensures adults with learning disabilities get the right support to live with independence and dignity whilst accessing STSFT acute services.

You will be a member of the adult acute disability service but work very closely with our community learning disabilities teams that supports people with a learning disability and/or autism based in a variety of settings within the trust.

The Learning Disability (LD) and Autism Liaison Services provides cover during the hours of 09.00 to 17.00 Monday to Friday but can sometimes arrange to offer support outside those hours by prior agreement in collaboration with community learning disabilities teams. We aim to contact all adult patients with a LD and/or Autism who is admitted to hospital within 24 hours, and work with the patient, carers, ward staff and community staff to ensure that this group of people are able to achie.ve the same health care outcomes as everybody else.

Main duties of the job

The Post Holder will:

Act as a positive role model in in the field of LD nursing through the provision of effective professional demonstration of competent, accountable clinical and most up to date practice.

Lead the LD&A acute services to give assurance to trust on compliance, training, legislation , policies and procedures are in place.

Support and line manage the acute team supporting wards and departments across the Trust.

Input into risk assessments and input into plans of care to meet the needs of vulnerable individuals in providing advice re assessments in relation to needs people with a LD and who are Autistic - such as use of the mental capacity assessment, safeguarding adults risk assessment, best interest decisions and deprivation of liberties assessments.

Assess and advise accordingly on issues surrounding the complex needs of people with LD/AS in relation to: Emergency and Elective Admissions, Discharge Planning, Access to investigations and diagnostics.

Give specialist advice in the management of the interface between hospital departments/wards, specialist community services, adult social care services and primary care advising when appropriate risk assessments and MDT overview is required in complex cases

Lead and manage mortality and LeDeR reviews in collaboration with LD&A community teams within the Trust and ensure learning embedded.

Lead with the community leads on the delivery of mandated LD&A training.
