Job summary
The Steps to Health service in Sunderland provide an innovative
and dynamic approach to individuals who are at risk or who have developed a long-term
condition and would benefit from exercise and rehabilitation to keep them well.
Education complements a programme of rehabilitation to provide knowledge and
support enabling patients to make healthy choices and self-care.
We would like to invite applications for a Registered Nurse to
join our team where you will play a key role working with specialists
supporting assessments of patients referred to the service and initiating a
plan of care.
You will be directly involved in education and in the delivery of
exercise classes which can be face to face or virtual.
You will be / have:
- Registered
Nurse.
- 1st Degree in
Nursing/ working towards.
- Experience of
working with patient/clients with a Long-Term Condition.
- Knowledge and
skills in using motivational interviewing and behaviour changes techniques.
- Excellent
interpersonal and communication skills.
- Experience of /
able to demonstrate ability to educate patients.
Main duties of the job
- Participate
in delivering training and education programmes to patient/clients specific to
their diagnosis.
- Support patient/clients to develop
self-care strategies and signpost to relevant agencies as required.
- Support the specialist
nursing/therapy team by completing identified intervention and review to enable effective clinical management of
a defined Long-Term Condition.
- Caseload management and
participation in clinical audit.
- Multi-disciplinary team working.
