inclusion nurse practitioner
We have an
excellent opportunity for an experienced, motivated individual to join our
team. You will be predominantly based at Basis centre in Sunderland but will be
expected to work across Sunderland as needed.
We are looking
for individuals with strong communication and interpersonal skills, highly
motivated with initiative. If you have a desire to reach out and improve the
care of our homelessness population we need you as part of our expanding
Transfer of Care Hub Team.
Main duties of the job
The post will work over Monday-Friday
covering the Sunderland geographical area, working with the homeless community
across the City.
The postholder will work with key
partners across the system including health, local authority, police and
ambulance services as well as the voluntary sector.
The postholder will deliver care to
homeless patients from designated settings across the city, supported by the GP
community and the Community Division structures, as well as signpost and advocate,
when necessary, to ensure homeless individuals access and receive care.

Provide nurse support to the homeless community ensuring they have access to
healthcare services and that their health needs are met, supported and facilitated
where access to services is required. This may be as they step down from acute
care or whilst in the community.
Provide physical healthcare to the increasing number of people experiencing
homelessness in Sunderland, and support people to access other healthcare
services.
Work closely with South Tyneside and Sunderland NHS Foundation Trust (STSFT)
and general practices, including encouraging people to register with a general
practice where appropriate.
Provide follow up care where appropriate to people experiencing homelessness
recently discharged form hospital.
Work in partnership with members of the multidisciplinary team from Community
Services, as well as other partner organisations. The service will support people
experiencing homelessness through transitions between settings (such as the
street, hostels, Housing First and other supported housing, hospital, mental health
services, social care, residential or community drug and alcohol treatment, and
custody). The Homelessness Inclusion Nursing Service will work closely with the
Hospital Discharge Team, and the Homelessness Reduction Officer.
Develop, implement and evaluate a seamless specialist service, ensuring that
patients receive the highest standard of clinical care.
Act as a lead specialist and will demonstrate a high level of expertise within the
specialist service, providing specialist advice, education and support to health care
professionals and patients.
Responsible for contributing to the ongoing development of clinical practice,
research and standards of care within the service, including development of
policies, protocols and guidelines and contribute to service developments in
collaboration with multidisciplinary colleagues.
Ensure the safety and wellbeing of services users, and always work in a manner
that promotes dignity and human rights through the adoption of person-centred
care principles.
Work primarily with homeless clients and will provide clinical leadership.
Participate in and develop/review team initiatives and projects for the client group
