Job summary

This is an exciting opportunity for Band 2 Health Care Support Workers (HCSW) to work as part of the NEW Community Ready Unit at South Tyneside District Hospital. This is a 14 bedded unit designed to support patients who are experiencing a delay in their discharge home from hospital. The unit will sit under the Community Division in South Tyneside and Sunderland.

The units philosophy builds on the 'Home First ' principles as these patients are ready for home and no longer in need of acute hospital care. The team will support patients to increase their independence, supporting mobilisation, and independence with dressing and other activities of daily living whilst they wait to go home. The CRU will provide a homely environment as the patients transition from hospital to home therefore part of the care delivery will be provision of stimulating activities.

To assist the registered practitioner in providing high quality compassionate care and assistance to patients / relatives and their carers appropriate to their needs

Responsible for carrying out tasks as delegated by the registered practitioner who has overall accountability for delegating care

Essential

  • Care Certificate
  • Level 2/3 Health/Health and Social Care NVQ or committed to completing Care Certificate qualification on commencement to post

Desirable

  • Maths and English at level 1 or above (GCSE A to G or equivalent)

Physical Skills

Essential

  • Ability to assist patients to move as per Trust Moving and Handling Policy

Experience

Essential

  • Experience of working with people
  • Experience of recording information in a clinical / IT system

Desirable

  • Experience of working in a hospital / community / care setting as a paid carer.

Knowledge and Skills

Essential

  • Ability to prioritise tasks
  • Ability to communicate health related information in an understandable way to patients and colleagues, adapting communication style to meet the needs of the recipient.
  • Ability to communicate messages and instructions accurately to colleagues, both verbally and in writing.
  • Basic skills of empathy and reassurance.
  • Basic keyboard skills.
  • Ability to use basic email system and document clearly, using plain English.
  • Basic understanding of health and safety requirements in a patient environment.
  • General understanding of confidentiality requirements in a health care setting.
