Are you an inspiring and motivated leader? Are you committed to leading your nursing team to provide the highest standards of nursing care? If so, we have a fantastic opportunity for you to become a Clinical Team Manager to lead our Discharge Nurses within our Transfer of Care Hubs.

The post holder will be required to have continuous responsibility for the safe management, development, organisation, and delivery of patient care for safe and timely discharges.

If successful in the role you will be responsible to ensure high standards of care are maintained, in compliance with the Key Performance Indicators (KPI's) and the Transfer of Care objectives.

You should be a professionally credible first level registered nurse with substantial experience of care management within the complex Discharge processes and be able to demonstrate a thorough understanding of specific pathways and advocate where possible the Home First approach.

You will demonstrate a strong leadership approach continuously improving our service, by ensuring systems are in place to support clinical governance, quality monitoring and ongoing research and audit of practice.

This role is across both sites of South Tyneside and Sunderland Hospital and the successful candidate will be expected to work across both sites.

To work autonomously, assessing individual patient / client needs, initiating investigations, determining a plan of care and initiating appropriate holistic, research-based health information and care.
