full/part time Assistant Practitioners to work within the DN Teams based in
Sunderland.
The post holder will carry out
planned and unplanned health (nursing) interventions as designated by the lead
clinician, and will work both supervised and unsupervised in patients own
homes, residential, nursing and other intermediate care facilities, reporting
back patient progress and informing the lead clinician of any problems
The Assistant Practitioner will provide advanced support to the
registered nurse and will work within the defined Trust policies, procedures
and competencies and treatment plans for patients creating safe, caring
environment that provides privacy, dignity and respect for the patient
Main duties of the job
The post holder will carry out

Qualifications
Essential
- Diploma or equivalent appropriate qualifications (UK equivalent), e.g. foundation degree; or NVQ3 level qualification plus short courses or relevant experience to diploma level
Experience
Essential
- Substantial experience of working in a Health and/or Social Care setting
- Experience of working collaboratively with a wide range of staff
- Proven prior experience of working as a Band 3 HCA in community services
- Working knowledge of National Policies/Procedures across Health and Social Care and Local Implementation
Desirable
- Experience of multi-disciplinary working
- Previous experience as a Healthcare Assistant in Acute care and Community setting
Physical skills

- Able to move and handle safely without causing harm to themselves or others
