to join our Hospital at Home team as a trainee Advanced clinical practitioner.
This post provides a clear pathway of progression, training and career
structure for an ACP. The post will develop to a band 8a upon completion of a MSc
in advanced clinical practice including a non-medical prescribing
qualification.
The
post holder will work autonomously and be able to provide a high level of clinical
management and care to patients within the speciality. They will act as a role
model working with the MDT and key stakeholders.
Hospital
at Home work in partnership with our Recovery at Home teams are defined as a
safe alternative to NHS bedded care, we are expanding our virtual ward team in
line with the introduction of new clinical and community step up pathways as
well as established hospital step down pathways. The team delivers care closer
to home, with technology enabled remote monitoring, providing medical, nursing,
therapy and pharmacy optimisation and onward referrals.
MSc
in Advanced Clinical Practice is a work based, multi-professional programme.
This develops the requisite knowledge, skills, and behaviours in a range of
specialist practice against the 4 pillars of Advanced Practice.
The
post holder will work collaboratively with the MDT. The post holder will be
expected to cover days and weekend shifts as part of their role. There will be
also dedicated allocated time for management and continuous professional
development.
Main duties of the job
We
are looking for a forward thinking, well organised and motivated individual
with a positive attitude, and a strong interested in community delivered care.
The applicant will need to act as a role model delivering advanced practice and
clinical effectiveness in care. As a Trainee ACP in community services, you
will be supported in developing the advanced clinical skills necessary to
assess, diagnose, and manage a variety of conditions. This is a fantastic
opportunity for individuals eager to progress their careers and gain extensive
experience within a multidisciplinary and collaborative environment. You will
be supported by experienced clinicians with a structured training program with
both practical and theoretical components. Conduct patient assessments,
including taking comprehensive histories, physical examinations within the
community. Diagnose and manage a wide range of conditions including respiratory
and frailty. Make clinical decisions regarding patient care plans, including
prescribing medications where appropriate. Work closely with other healthcare
professionals in a collaborative, team-based approach. Participate in
continuous professional development, including attending relevant training
sessions and completing academic assignments.
