South Tyneside and Sunderland NHS Foundation Trust is a unique organisation delivering Acute and Community services. Our district nursing teams are integrated with Local Authority services and co-ordinate care for individuals to enable them to live healthily and independently in their own homes. We are seeking full time and part time highly motivated SP District Nurses, to join the District Nursing Teams across South Tyneside and Sunderland. You will be involved in the provision of high-quality nursing assessments and be responsible for the delivery of clinical care. The ability to communicate positively at all levels will be an essential attribute of the post holder, as well as a flexible self-confident approach to work. The post holder will have the knowledge and experience to provide a valuable contribution to the development of more flexible and innovative working both within the team and across organisations in the statutory and non-statutory sector.

Main duties of the job

You will be involved in the provision of high-quality nursing assessments and be responsible for the delivery of clinical care for a defined caseload of patients. The ability to communicate positively at all levels will be an essential attribute of the post holder, as well as a flexible self-confident approach to work. The post holder will have the knowledge and experience to provide a valuable contribution to the development of more flexible and innovative working both within the team and across organisations in the statutory and non-statutory sector.
