Job summary

We are looking to recruit a caring, compassionate nurse to a Band 5 post in the Sunderland Out of Hours Palliative Care Team 28 hpw. The Team are part of an Integrated Specialist Palliative Care Team, and provide high quality palliative and end of life care in the out of hour’s period which is timely and meets the needs of patients, carers and families.

You will work as part of a team and alongside the nursing sister/charge nurse, providing skilled palliative care to those with life limiting illnesses within their own home. Provide skilled emotional support to patient and carers.

You will be a key member, providing palliative care, advice & support to patients, carers and professionals. Work collaboratively with multi professional teams to ensure safe & effective practice.

Main duties of the job

Main aims of the Service are to:

Manage pain & other symptoms to enable patients to be cared for at home.

Manage & administer complex medications, ensuring patient safety.

Provide choice & support to patients and families achieving preferred place of care/death.

Prevent unnecessary admission to hospital.

Facilitate rapid discharge from hospital for patients at end of life.

Provide advice & support to patients’, carers and other professionals in relation to palliative & end of life care issues.

You will be expected to maintain agreed standards of care, directly protecting & promoting patients dignity, choices, self esteem and well being at all times.

Two-shift system between the hours of 16:00-08:45 Monday to Friday and a three-shift system weekends and bank holidays. Shift patterns are reviewed regularly and may be subject to periodic change.

The nominal base for this role will be St Benedict’s Hospice and Centre for Specialist Palliative Care in Sunderland, however you may be required to work across the whole of Sunderland and S/T. We offer excellent working conditions and a supportive culture promoting Continuous Professional Development and Life Long Learning.

  • Diploma in health-related study or equivalent appropriate qualification.

  • Degree in nursing studies or healthcare-related subject (UK equivalent).
  • Mentorship Certificate/Practice Assessor training or demonstrable equivalent experience.

Experience

  • Experience of working in an acute hospital or community healthcare setting.
  • Post-registration experience in palliative/cancer care.

Desirable

  • Recent experience of working as a registered nurse.
  • Community experience.

Skills and knowledge

  • Ability to provide and receive complex, sensitive or contentious information.
  • Able to work without supervision.
  • Competent in the use of IT such as Microsoft Office.
  • Evidence of CPD.
