This is a part time vacancy for 27 hours per week.

The Discharge Lounge welcomes applications for experienced Band 5 Staff Nurses to come and join our team. The Discharge Lounge is an integral part of patient flow bringing patients awaiting discharge down to the lounge whilst awaiting medication or transport freeing up an acute bed earlier in the day. The job is fast paced, applicants will need excellent time management and prioritising skills to co ordinate safe and timely discharges from the Lounge. The ideal candidates will be able to work under pressure, have excellent communication skills and a can do attitude. They will work with the multi discipliniary team including transfer of care hub, site manager, social workers, rapid response, recovery at home, pharmacy. Prioritising transport for safe discharge and coordinating packages of care to ensure patient safety. Applicants should be familiar with red to green and the home first approach. This post is predominantly for Sunderland Discharge Lounge however staff may be expected to cover at South Tyneside Discharge Lounge.

Main duties of the job

You will be working closely with site managers and transfer of care hub to pull planned discharges to the Discharge Lounge to ensure beds are free earlier in the day. You will develop a rapport with the ward staff and be available for advice to staff.

The relevant transport will be arranged, ensuring relevant assessments have been completed prior to discharge and all access issues are resolved. Transport is to be prioritised and co ordinated with external agencies to ensure safety of patients.

Ensuring clear concise documentation at all times.

The Discharge Lounge is open 8-8 Monday to Saturday and 8-6 Sunday.
