are integrated with Local Authority services and co-ordinate care for
individuals to enable them to live healthily and independently in their own
homes. We are seeking full time and part time highly motivated SP District
Nurses, to join the Integrated Care Community Teams across South Tyneside and
Sunderland. You will be involved in the provision of high quality nursing assessments
and be responsible for the delivery of clinical care. The ability to
communicate positively at all levels will be an essential attribute of the post
holder, as well as a flexible self-confident approach to work. The post holder
will have the knowledge and experience to provide a valuable contribution to
the development of more flexible and innovative working both within the team
and across organisations in the statutory and non-statutory sector. In return

within the appraisal process. A recognised qualification relevant to the
speciality is essential. Car driver /access to a car is essential.
Main duties of the job
The post holder will work within an integrated team to proactively manage,
co-ordinate and support an identified caseload of patients
You will be an excellent communicator
and work well both individually and within a team. You will be patient focused
and have the ability to undertake clinical assessments; start, plan and
evaluate care. You will also have organisational and time management skills.
You will demonstrate leadership skills across the inter-professional team.
A
recognised qualification relevant to the speciality is essential as is a car
driver /access to a car is essential.
