Do you want to
work autonomously as a nurse practitioner in an Intermediate Care and
Rehabilitation Unit? There is an opportunity to apply for a full time, 30 hours,
Nurse Practitioner role. ICAR is a 24
bed nurse led unit based at Houghton-le-Spring Primary Care Centre, which have
a team of Nurse Practitioners who manage patient care and lead on the development
patient pathways. The wider team consists of NPs, Medical cover, Nurses,
Physiotherapists and Occupational Therapists. The Nurse Practitioners work
across 6 days coordinating the cover between themselves. Working times can be
flexible.
You will be
encouraged to develop professionally and take part in clinical supervision.
Parking at ICAR is free and there are electric charging points.

Main duties of the job

To
assess patients when nursing staff identify a determination in health

participate in MDT meetings with the consultant

take part in team huddles and handovers when patient care needs to be updated

coordinate training for staff to update on clinical skills or systems

take a lead role in clinical developments, change in clinical guidance and/or
admission criteria changes

review policy/guidelines linked to clinical care

be an active part of the Trust, being involved in Team meetings, governance and
