We want to recruit senior clinicians who can support and empower patients within the frailty agenda.

We are looking for highly skilled, enthusiastic, self-motivated senior nurse to join our Frailty Team based at Clarendon, Hebburn.

The role of the Nurse Practitioner is an essential part of the Frailty Team - working closely with GPs in the locality.

You will be responsible for patients living in the community who are identified as having moderate to severe frailty, multiple co morbidities, complex needs or who are identified as high intensity servicer users.

Main duties of the job

As a Nurse Practitioner in the Community FrailtyTeam,you will have a great working knowledge of care ofpatients with frailty and complex health needs.

You willwork autonomously,beadynamic,approachableand motivatedleader and clinicianand you willbe responsible forensuringhigh standardsaremaintained.

Youwillhave a desire for continualpersonal and serviceimprovement.

Core working hours 8-30-5pm Monday Friday.

  • Proven interpersonal skills.
