Job summary
We are looking for a highly motivated Nurse Practitioner to work within the
Sunderland Community Frailty Team in Sunderland. The team consists of Band 7
and Band 6 Nurse Practitioners who are based in each of our six Community
Integrated Teams. You will be responsible
for out of hospital care assessment and co-ordination of care and services to
people who live in their own home or nursing or residential care and are
identified as having moderate to severe frailty, multiple co-morbidities,
complex needs or who are identified as high intensity service users. You will work as a member of the a Multidisciplinary Team and you will be an
autonomous practitioner.
You will be responsible for pro-actively case finding patients with moderate
to severe frailty and undertaking comprehensive assessment, personalised care
planning, providing support and advice to enhance self-care and improve quality
of life for older people and reducing level of frailty wherever possible.
You will work autonomously, assessing individual
patient needs, initiating investigations, with support determining a
differential diagnosis and initiating appropriate holistic, evidence based
treatment and care. You will refer patients who require medical intervention to
the appropriate services.

Main duties of the job
As a Junior
Nurse Practitioner in the Community Frailty Team you will
be an experienced Registered Nurse, who preferably has experience at
Band 6 level, you will have a great working knowledge of care of older people
and patients with frailty and complex health needs and you will be a confident
autonomous professional. You will be a dynamic, approachable
and self motivated individual, you will be responsible for ensuring high
standards are maintained and you will have a desire for continual improvement.
You will have:
- A full UK driving liscence and access to a car.
- Registered Nurse with NMC.
- A nursing or related degree, or working
towards.
- Advanced
clinical assessment skills accreditation or willingness to work towards.
- Non medical
prescribing qualification or willing to work towards extended prescribing
skills.
- Advanced
decision making skills.
- Experience
of leading service developments and managing change.
- Car driver
with access to transportation and a full UK driving license.
